埋线联合耳穴埋豆治疗肥胖型PCOS不孕症的效果及对患者肠道菌群、糖脂代谢的影响

目的 探讨埋线联合耳穴埋豆治疗肥胖型多囊卵巢综合征(PCOS)不孕症的效果及对患者肠道菌群、糖脂代谢的影响。方法 选取2019年11月至2021年5月在河北省沧州中西医结合医院就诊的134例肥胖型PCOS不孕症患者作为研究对象,根据治疗方案将患者分为对照组(n=67)及观察组(n=67)。对照组采用埋线治疗,观察组在对照组基础上联合耳穴埋豆进行治疗。比较两组治疗前后肠道菌群、糖脂代谢及胰岛素抵抗情况。结果 治疗3个月后,观察组肠杆菌菌落数、甘油三酯(TG)、血清总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)水平均低于对照组,观察组乳酸杆菌、双歧杆菌菌落数及高密度脂蛋白胆固醇(HDL-C)水平均较对照组高(P<0.05);治疗后两组空腹血糖(FBG)、空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR)水平均随时间发展而呈下降趋势,且观察组下降程度大于对照组(P<0.05)。结论 采用埋线联合耳穴埋豆治疗方式,可有效改善肥胖型PCOS不孕症患者肠道微生物稳态失衡,并有助于改善患者糖脂代谢紊乱,从而对患者临床症状改善有所帮助。

Objective To investigate the effect of catgut embedding combined with ear acupoint embedding in the treatment for infertility in obese polycystic ovary syndrome(PCOS)and its effect on intestinal flora and glucose and lipid metabolism in patients.Methods A total of 134 cases of obese PCOS infertility who were treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine,Hebei Province from November 2019 to May 2021 were selected as the included subjects.According to the selected treatment plan,the patients were divided into the control group(n=67)and the observation group(n=67).The control group was treated with catgut embedding,and the observation group was treated with ear acupoint embedding on the basis of the control group.The intestinal flora,glucose and lipid metabolism and insulin resistance were compared between the two groups before and after treatment.Results After 3 months of treatment,the colony counts of Enterobacteriaceae,triglycerides(TG),serum total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)levels in the observation group were lower than those in the control group,the colony counts of Lactobacillus,Bifidobacterium and high-density lipoprotein cholesterol(HDL-C)levels were higher than those before treatment,and the observation group was higher than the control group(P<0.05);after treatment,the levels of fasting blood sugar(FBG),fasting insulin(FINS)and insulin resistance index(HOMA-IR)in the two groups decreased with time,and the degree of decrease in the observation group was greater than that in the control group(P<0.05).Conclusions The treatment of catgut embedding combined with ear acupoint embedding can effectively improve the imbalance of intestinal microbial homeostasis in obese PCOS infertility patients,and help improve the disorder of glucose and lipid metabolism in patients,thereby helping to improve the clinical symptoms of patients.
