半夏白术天麻汤联合西药治疗急性缺血性脑卒中临床观察

Banxia Baizhu Tianma Decoction Combined with Western Medicine in the Treatment of Cerebral Apoplexy

目的 探讨脑卒中患者应用半夏白术天麻汤联合西药治疗的临床效果。方法 选取抚州市东乡区第二人民医院2019年11月—2021年11月收治的共88例急性缺血性脑卒中(AIS)患者作为研究对象,按照随机数字表法分为观察组与对照组,各44例。对照组予以单唾液酸四己糖神经节苷脂钠注射液治疗;观察组加用半夏白术天麻汤治疗,均连续2周。对2组临床疗效、血清学指标、脑血流动力学指标及不良反应进行比较。结果 观察组临床总有效率为95.45%(42/44),高于对照组的79.55%(35/44)(P<0.05);观察组患侧大脑中动脉(MCA)平均流速(Vm)、峰流速(Vp)高于对照组,差异有统计学意义(P<0.05);超敏C反应蛋白(hs-CRP)、D-二聚体(DD)及MCA平均流速对称性差值(DVm)、峰流速对称性差值(DVp)低于对照组,差异有统计学意义(P<0.05)。治疗期间2组未见明显不良反应。结论 AIS患者在应用单唾液酸四己糖神经节苷脂钠注射液治疗基础上联合半夏白术天麻汤能够提升临床效果,改善血清学指标和脑血流动力学指标,安全可靠,是一种较为理想的治疗方案。

Objective To investigate the clinical effect of Banxia Baizhu Tianma decoction combined with Western medicine in the treatment of stroke patients.Methods A total of 88 patients with acute ischemic stroke(AIS)in The Second People’s Hospital of Dongxiang District from November 2019 to November 2021 were divided into two groups according to the random number table method,with 44 cases in each group.The control group was treated with ganglioside,and the observation group was additionally treated with Banxia Baizhu Tianma decoction for 2 consecutive weeks.The clinical efficacy,serological indexes,cerebral hemodynamic indexes and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.45%(42/44),which was higher than 79.55%(35/44)of control group(P<0.05).The mean velocity(Vm)and peak velocity(Vp)of the affected middle cerebral artery(MCA)in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The average velocity symmetry difference(DVm)and peak velocity symmetry difference(DVp)of hypersensitive C-reactive protein(hs-CRP),D-dimer(DD)and MCA were lower than those of control group,and the difference was statistically significant(P<0.05).No obvious adverse reactions were observed in the two groups during treatment.Conclusion The application of ganglioside in AIS patients combined with Banxia Baizhu Tianma decoction can improve clinical efficacy,improve serological indexes and cerebral hemodynamics,which is safe and reliable,and is an ideal treatment plan.
