五子衍宗丸加减治疗精索静脉曲张致少弱畸精子症的临床应用观察

Clinical Observation of Wuziyanzong Pills in the Treatment of Oligoasthenospermia Caused by Varicocele

探讨五子衍宗丸加减组方联合显微精索手术治疗精索静脉曲张(VC)所导致的少、弱、畸精子症的临床应用观察。选取2019年2月到2022年6月期间56例因VC导致精液质量异常(少、弱、畸)而不育的患者,随机分为观察组和对照组各28例,观察组只做显微精索静脉结扎术;对照组在显微精索静脉结扎术后第五天服用以五子衍宗丸为基础方进行加减组方的汤剂。观察组和对照组患者治疗后与治疗前相比较精子前向运动率、精子浓度、精子数量、形态正常精子比率均明显提高,DNA碎片率(DFI)明显降低,差异具有统计学意义(P<0.05);观察组和对照组3个月治疗后精子数、精子浓度、精子前向运动率、形态正常精子比、DFI相比较,对照组改善优于观察组,差异具有统计学意义(P<0.05);两组治疗后3个月在自然怀孕率方面差异无统计学意义(P>0.05)。五子衍宗丸为基础方随症加减进行组方联合显微精索静脉结扎术能明显改善精液质量,降低精子DNA碎片率,对VC导致的少、弱、畸精子症治疗效果满意。

To explore the clinical application of Wuziyanzong pills combined with microspermatic cord surgery in the treatment of oligozoospermia,asthenospermia and teratospermia caused by varicocele(VC),56 infertile patients with abnormal semen quality caused by VC from February 2019 to June 2022 were randomly divided into observation group(n=28)and control group(n=28).The observation group was only treated with microscopic spermatic vein ligation,and the control group was treated with Wuziyanzong pills on the fifth day after microspermatic vein ligation.After treatment,the sperm forward motility rate,sperm concentration,sperm count and sperm ratio with normal morphology in the observation group and the control group were significantly higher than those before treatment,while the DNA fragmentation rate(DFI)decreased significantly.The sperm count,sperm concentration,sperm forward movement rate,sperm ratio with normal morphology and DFI in the observation group were better than those in the observation group 3 months after treatment,but there was no significant difference in the natural pregnancy rate between the two groups 3 months after treatment.Wuziyanzong pills combined with microscopic spermatic vein ligation can improve semen quality and reduce the rate of sperm DNA fragments,and are effective in the treatment of oligozoospermia,asthenospermia and abnormal sperm caused by VC.
