中青年结直肠癌患者社会疏离现状及其影响因素分析

Current situation of social alienation and its influencing factors among young and middle-aged patients with colorectal cancer

目的探讨中青年结直肠癌患者社会疏离现状及其影响因素,为临床干预提供依据。方法采用便利抽样法,选取2022年1—12月于滨州医学院附属医院复查的130例中青年结直肠癌患者为研究对象。采用一般资料调查表、一般疏离感量表、恐惧疾病进展量表简表、社会影响量表对患者进行调查。采用多重线性回归分析进行影响因素分析。结果本研究共发放问卷130份,回收有效问卷123份,有效回收率为94.62%。130例中青年结直肠癌患者的一般疏离感量表得分为(39.75±7.57)分,恐惧疾病进展量表简表得分为(37.87±7.13)分,社会影响量表得分为(50.02±13.56)分。多重线性回归分析结果显示,性别、年龄、手术距今时间、是否造口、恐惧疾病进展、病耻感是中青年结直肠癌患者社会疏离的影响因素(P<0.05)。结论医护人员应重点关注女性、年龄较小、造口、手术时间较短、恐惧疾病进展和病耻感水平较高的结直肠癌患者,开展干预性研究,从而降低患者的社会疏离,帮助患者早日回归社会。

Objective To explore the status quo and influencing factors of social alienation in young and middle-aged patients with colorectal cancer,so as to provide evidence for clinical intervention.Methods A total of 130 young and middle-aged colorectal cancer patients who were reexamined in Binzhou Medical University Hospital from January to December 2022 were selected as research objects by the convenient sampling method.The general data questionnaire,General Alienation Scale(GAS),Fear of Progression Questionnaire-Short Form(FoP-Q-SF)and Social Impact Scale(SIS)were used to investigate the patients.Multiple linear regression analysis was used to analyze the influencing factors.Results A total of 130 questionnaires were sent out in this study,and 123 were effectively collected,with an effective recovery rate of 94.62%.The scores of 130 young and middle-aged patients with colorectal cancer were(39.75±7.57)on General Alienation Scale,(37.87±7.13)on Fear of Progression Questionnaire-Short Form and(50.02±13.56)on Social Impact Scale.The results of multiple linear regression analysis showed that gender,age,time since surgery,stomy or not,fear of disease progression and stigma were the influential factors of social distancing in young and middle-aged colorectal cancer patients(P<0.05).Conclusions Medical staff should focus on women,younger age,ostomy,shorter operation time,fear of disease progression and high levels of stigma in colorectal cancer patients,to carry out intervention studies,so as toreduce patients'social alienation and help patients return to society as soon as possible.
