血管黏附蛋白-1联合多指标对妊娠期糖尿病的诊断价值

Value of VAP-1 combined with multiple indicators in the diagnosis of gestational diabetes

目的 分析血管黏附蛋白-1(VAP-1)联合多指标在妊娠期糖尿病(GDM)中的诊断价值。方法分别选取100例GDM和糖耐量正常单胎孕妇作为观察组和对照组,收集其孕9周到13周+6期间外周血VAP-1、中性粒细胞/淋巴细胞比值(NLR)、超敏C反应蛋白(hs-CRP)、空腹血糖(FBG)和糖化血红蛋白(HbA1c)结果 ,比较2组各指标间差异,分析外周血VAP-1与其他指标相关性,并探讨各指标及联合指标对GDM的诊断价值。结果 观察组孕妇外周血VAP-1、NLR、hs-CRP、FBG和HbA1c水平(3.4±1.4,3.5±1.9,3.7±1.2,5.2±0.7,5.35±0.37)均高于对照组(2.2±0.7,2.7±0.8,2.2±0.7,4.7±0.4,5.12±0.28),2组间比较有统计学意义(t=25.853,25.962,13.058,22.354,5.198,P均<0.05)。Spearman相关性分析发现,外周血VAP-1与NLR、hsCRP、FBG、HbA1c间呈正相关(r=0.393,0.315,0.443,0.466,P均<0.05)。绘制ROC曲线发现,VAP-1、NLR、hsCRP、FBG、HbA1c均可诊断GDM (P均<0.05),联合诊断曲线下面积(AUC)=0.925,敏感度88.4%,特异度94.2%。结论 外周血VAP-1等多指标在GDM中高表达,各指标均对可诊断GDM,以联合指标效果最佳。

Objective To analyze the diagnostic value of vascular adhesion protein-1(VAP-1)combined with multiple indicators in gestational diabetes mellitus(GDM).Methods One hundred cases of GDM singleton pregnant women were treated as the observation group,and 100 cases of normal glucose tolerance singleton pregnant women as the control group.Peripheral blood VAP-1,neutrophil to lymphocyte ratio(NLR),high sensitivity C-reactive protein(hs-CRP),fasting blood glucose(FBG),glycated hemoglobin(HbA1c)were collected in all pregnant women from 9 weeks to 13 weeks of gestation plus 6 period.The levels of those indexes were compared between the two groups.The correlation between peripheral blood VAP-1 and other laboratory indexes was analyzed,and the diagnostic value of various indicators and combined indicators were explored for GDM.Results The levels of peripheral blood VAP-1,NLR,hs CRP,FBG,and HbA1c of pregnant women in the observation group(3.4±1.4,3.5±1.9,3.7±1.2,5.2±0.7,5.35±0.37)were significantly higher than those in the control group(2.2±0.7,2.7±0.8,2.2±0.7,4.7±0.4,5.12±0.28),and there was statistical significance between the two groups(t=25.853,25.962,13.058,22.354,5.198,P<0.05).Spearman correlation analysis found a positive correlation between peripheral blood VAP-1,NLR,hs CRP,FBG,and HbA1c(r=0.393,0.315,0.443,0.466,P<0.05).Drawing the ROC curve,it was found that VAP-1,NLR,hs CRP,FBG,and HbA1c can all be used to diagnose GDM(P<0.05),and the combined diagnostic efficacy of all indicators is the best.Conclusions Multiple indicators such as VAP-1 in peripheral blood are highly expressed in GDM,and all indicators are suitable for diagnosing GDM.The combination of these indicators has the best effect.
