枇杷清肺饮联合加味金黄膏外敷治疗肺胃热盛型痤疮的疗效观察

Clinical Observation of Loquat Qingfei Decoction Combined with External Application of Jiawei Jinhuang Ointment in Acne with Heat Excessive Lung and Stomach

目的 观察枇杷清肺饮联合加味金黄膏外敷治疗肺胃热盛型痤疮的临床效果。方法 选取本院皮肤科2020年8月—10月收治的68例肺胃热盛型痤疮患者作为研究对象,按随机数字表法将患者分为对照组与联合组每组34例。对照组给予阿达帕林凝胶外用联合米诺环素胶囊口服治疗,联合组在对照组的治疗基础上采用枇杷清肺饮联合加味金黄膏外敷治疗。观察2组中医症状积分、痤疮复发率及不良反应发生情况。结果 治疗后,联合组的临床总有效率明显高于对照组(94.1%比61.8%,P<0.05);治疗前2组中医症状积分(面部潮红、丘疹脓疱、脱屑瘙痒、焮热肿痛)比较差异均无统计学意义(均P>0.05),治疗后联合组各项中医症状积分均明显低于对照组[面部潮红(分):1.23±0.32比2.24±0.29,丘疹脓疱(分):0.65±0.32比2.14±0.33,脱屑瘙痒(分):0.99±0.32比1.52±0.34,焮热肿痛(分):0.97±0.31比2.36±0.54]均优于正常组(P<0.05);对照组复发率高于联合组(57.1%比9.1%,P<0.05);且不良反应也高于联合组(32.4%比11.8%,P<0.05)。结论 枇杷清肺饮联合加味金黄膏能改善肺胃热盛型痤疮患者的临床症状,减少复发率和不良反应,值得临床借鉴。

Objective In order to observe the effect of Loquat Qingfei decoction combined with external application of Jiawei Jinhuang ointment to treat acne caused by heat of lung and stomach.Methods A total of 68 patients with pulmonary and stomach fever acne who were admitted to the department of dermatology in our hospital from August 2020 to October 2020 were selected as research objects.According to the random number table method,they were divided into the control group and the combination group,including 34 cases in each group.The control group was given adapalene gel topical combined with minocycline capsules for oral treatment,and on the basis of the control group,the combination group was treated with loquat Qingfei decoction and Jiawei Jinhuang ointment external application.The traditional Chinese medicine(TCM)symptom score,acne recurrence rate and adverse reactions were observed in the two group.Results After treatment,the total clinical effective rate of the combination group(94.1%)was higher than that of the control group(61.8%,P<0.05).There were no differences in TCM symptom score between two groups before treatment(P>0.05).After treatment,the facial flushing[(2.24±0.29)points],papules and pustules[(2.14±0.33)points],desquamation and itching[(1.52±0.34)points]in the combination group were better than those in the control group[(1.23±0.32)points,(0.65±0.32)points,(0.99±0.32)points,(0.97±0.31)minutes](P<0.05);the recurrence rate of the control group(57.1%)was higher than that of the combination group(9.1%,P<0.05);and the adverse reaction(32.4%)was also higher than that of the combination group(11.8%,P<0.05).Conclusion Loquat Qingfei decoction combined with Jiawei Jinhuang ointment can improve the clinical symptoms of acne patients with heat excessive lung and stomach,with less recurrence rate and adverse reactions,and is worthy of clinical promotion.
