脑电图监测对轻症病毒性脑膜炎的辅助诊断价值

Diagnostic value of electroencephalography monitoring in mild viral meningitis

目的探讨脑电图监测对轻症病毒性脑膜炎的辅助诊断价值。方法选取邢台市人民医院神经内科2022年1月至2023年1月轻症病毒性脑膜炎表现的53例患者资料进行回顾性研究,其中男25例,女28例,年龄17~67岁。患者就诊时主要表现为头痛不适,病程6 h~15 d。所有患者行脑电图、影像学及脑脊液检查,观察患者2小时脑电图监测结果,腰穿检查结果、头颅影像学检查结果以及抗病毒治疗后预后转归情况。结果所有患者进行了头颅CT检查,均未见异常。53例患者中,脑电图正常25例,异常28例,异常率52.8%,主要表现为前头部为著的θ慢波增多,少部分患者表现为枕区α节律减慢。脑脊液总体异常率为56.6%。脑电图异常率与脑脊液总体异常率比较,差异无统计学意义(52.8%比56.6%,P>0.05),均高于脑脊液常规(30.2%)及细胞学(32.1%)异常率(P<0.05)。所有患者预后良好。结论脑电图监测对轻症病毒性脑膜炎具有重要的辅助诊断价值。

Objective To explore the diagnostic value of electroencephalography(EEG)monitoring in mild viral meningitis.Methods The data of 53 patients with mild viral meningitis from January 2022 to January 2023 in the Department of Neurology of Xingtai People's Hospital were retrospectively studied,including 25 males and 28 females,17-67 years old.The patients mainly presented with headache,with a course of 6 hours to 15 days.All patients underwent electroencephalography,imaging and cerebrospinal fluid(CSF)examination,and the results of 2-hour EEG monitoring,CSF and imaging,and prognosis after antiviral treatment were observed.Results Head CT examination was performed in all patients and no abnormality was found.Among the 53 patients,EEG was normal in 25 cases and abnormal in 28 cases,with an abnormal rate of 52.8%.The main manifestations were increasedθslow wave in the anterior head,and a few patients showed slowerαrhythm in the occipital region.The overall abnormal rate of CSF was 56.6%.There was no significant difference between the abnormal rate of EEG and the overall abnormal rate of CSF(52.8%vs.56.6%,P>0.05),but it was higher than the abnormal rate of CSF routine(30.2%)and cytology(32.1%)(P<0.05).All patients had good prognosis.Conclusion EEG monitoring has important diagnostic value in mild viral meningitis.
