利奈唑胺治疗重症结核性脑膜炎疗效及对脑脊液生化指标的影响

Effect of Linezolid in the treatment of severe tuberculous meningitis and its effects on biochemical indexes of cerebrospinal fluid

摘要 目的观察利奈唑胺治疗重症结核性脑膜炎(TBM)疗效及对脑脊液生化指标的影响.方法选取上饶市人民医院2021年2月-2023年1月收治的80例重症TBM患者,用随机数字表法分为对照组、试验组,每组40例;对照组予以经典四联抗痨治疗,试验组在对照组基础上加用利奈唑胺治疗;对比两组临床疗效、脑脊液生化指标及不良反应发生情况.结果试验组总有效率较对照组高,差异有统计学意义(P<0.05);两组治疗1个疗程后的脑脊液糖/同步血糖比值较治疗前升高,白细胞计数、蛋白定量浓度较治疗前降低,且试验组各指标水平变化显著,差异有统计学意义(P<0.05);两组不良反应总发生率对比,差异无统计学意义(P>0.05).结论重症TBM患者采用利奈唑胺治疗效果较好,可改善脑脊液生化指标,且药物安全性好. Objective To observe the efficacy of linezolid in the treatment of severe tuberculous meningitis(TBM)and its effects on cerebrospinal fluid biochemical indices.Methods 80 patients with severe TBM admitted to the hospital from February 2021 to January 2023 were selected and divided into control group and experimental group by random number table method,with 40 cases in each group.The control group was treated with classical quadruple antituberculosis therapy,and the experimental group was treated with nezolamide.The clinical efficacy,cerebrospinal fluid biochemical indexes and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of experimental group was higher than that of control group,the difference was statistically significant(P<0.05).After 1 course of treatment,the ratio of cerebrospinal fluid sugar to synchronous blood glucose was increased in both groups,while the white blood cell count and quantitative protein concentration were decreased compared with that before treatment,and the indexes of experimental groups were significantly changed,with statistical significance(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment effect of linezolid in severe TBM patients is better,which can improve the biochemical indexes of cerebrospinal fluid and has good safety.
