超声引导下经筋刀联合关节囊内液压扩张术治疗肩周炎的临床效果

Clinical Effect of Ultrasound-guided Tendon Knife Combined with Intracapsular Hydraulic Dilation in the Treatment of Scapulohumeral Periarthritis

目的:探讨超声引导下经筋刀联合关节囊内液压扩张术治疗肩周炎的临床效果。方法:对2020年8月—2021年8月于九江市第一人民医院中西医结合风湿免疫科确诊为肩周炎的80例患者资料进行回顾性分析,根据治疗方式差异分为对照组及观察组,各40例。对照组采用关节囊内液压扩张术治疗,观察组采用超声引导下经筋刀联合关节囊内液压扩张术治疗。比较两组患者疼痛指标、肩关节活动度、不良事件发生率。结果:治疗后,观察组视觉模拟评分法评分低于对照组,差异有统计学意义(P=0.041);观察组疼痛度、关节活动度、日常生活能力及肌力评分均高于对照组,差异有统计学意义(P<0.001);两组不良事件发生率比较,差异无统计学意义(P>0.05)。结论:超声引导下经筋刀联合关节囊内液压扩张术在肩周炎患者中治疗效果显著,可以减轻患者疼痛程度,改善肩关节活动度,且安全性较高,值得推广。

Objective:To explore the clinical effect of ultrasound-guided tendon knife combined with intra-capsular hydraulic expansion in the treatment of scapulohumeral periarthritis.Methods:A total of 80 patients datas diagnosed with scapulohumeral periarthritis were retrospectively analyzed at Jiujiang First People's Hospital Rheumatology Immunology Department from August 2020 to August 2021,they were divided into control group and observation group according to the differences in treatment methods,with 40 cases in each group.The control group was treated with intra-articular hydraulic dilatation,and the observation group was treated with ultrasound-guided tendon knife combined with intra-articular hydraulic dilation.The pain index,shoulder joint range of motion,and incidence of adverse events were compared between the two groups.Results:After treatment,the visual analog scale score of the observation group was lower than that of the control group,and the difference was statistically significant(P=0.041);the pain,joint mobility,daily living ability and muscle strength scores of the observation group were all higher than those of the control group,The difference was statistically significant(P<0.001);there was no statistically significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion:Ultrasound-guided tendon knife combined with intra-capsular hydraulic expansion has a significant therapeutic effect on patients with scapulohumeral periarthritis.It can reduce pain and improve the range of motion of the shoulder joint.It is safe and worthy of promotion.
