5种基因mRNA在结直肠癌患者粪便中的表达及临床意义 被引量:1

摘要 [目的]探讨5种基因(MTHFR、CK20、MMP-11、CD24和COX-2)mRNA在结直肠癌患者粪便中的表达及临床意义。[方法]选取100例结直肠癌患者作为研究组,100例非结直肠癌患者作为对照组,分析2组研究对象粪便MTHFR、CK20、MMP-11、CD24和COX-2mRNA的表达情况及诊断敏感性,以及5种基因与结肠癌临床病理指标间的相关性。[结果]MTHFR、CK20、MMP-11、CD24和COX-2mRNA在病灶及粪便中均有表达,与粪便表达同源性分别为92.0%、81.0%、92.0%、92.0%、84.0%;研究组MTHFR、CK20、MMP-11、CD24和COX-2mRNA表达明显高于对照组(P<0.05);粪便中MTHFR、CK20、MMP-11、CD24和COX-2mRNA表达诊断结直肠癌准确性分别为76.5%、71.0%、74.0%、69.5%、69.5%,联合检测准确性为87.5%;肿瘤大小5 cm以上COX-2mRNA、CK20 mRNA表达高于5 cm(P<0.05);Dukes分期C、D分期CK20 mRNA高于Dukes分期A、B分期(P<0.05);存在肿瘤大小5 cm以上、Dukes分期(C、D)、转移情况患者CD24 mRNA、MTHFRmRNA、MMP-11 mRNA表达高于5 cm以下、Dukes分期(A、B)、无转移患者(P<0.05)。[结论]5种基因在粪便中的表达与原发灶表达具有同源性,且与患者临床病理分期、肿瘤大小、是否转移存在密切联系,通过对粪便中MTHFR、CK20、MMP-11、CD24和COX-2mRNA进行检测,可以有效诊断结直肠癌发生及发展情况,联合检测其应用效能越高。 [Objective]To investigate the expression of mRNA of five genes in stool of patients with colorectal cancer and its clinical significance.[Methods]A total of 100 patients with colorectal cancer admitted to our hospital from January 2019 to May 2020 were selected as the study group and 100 non-colorectal cancer patients as the control group.The expressions and diagnostic sensitivity of fecal MTHFR,CK20,MMP-11,CD24 and COX-2mRNA of the two groups were analyzed,and the correlation between five genes and clinicopathological indicators of colon cancer was analyzed.[Results]MTHFR,CK20,MMP-11,CD24 and COX-2mRNA were all expressed in lesions and feces,and their homology with feces expression was 92.0%,81.0%,92.0%,92.0% and 84.0%,respectively.The mRNA expressions of MTHFR,CK20,MMP-11,CD24 and COX-2in the colorectal cancer group were significantly higher than those in the healthy control group(P<0.05).The accuracy of MTHFR,CK20,MMP-11,CD24 and COX-2mRNA expression in the stool for the diagnosis of colorectal cancer was 76.5%,71.0%,74.0%,69.5%,69.5%,respectively,and the combined detection accuracy was 87.5%.The expression of COX-2mRNA and CK20 mRNA in the tumor size above 5 cm was higher than that in the tumor size above 5 cm(P<0.05).mRNA levels of Dukes stage C and D CK20 were higher than those of Dukes stage A and B(P<0.05).There was tumor size of more than 5 cm,Dukes stage(C,D),metastasis patient of CD24 mRNA,MTHFRmRNA,MMP-11 mRNA expression higher than that of less than 5 cm,Dukes stage(A,B),no metastasis patient(P<0.05).[Conclusion]In the faeces of five genes expression and primary foci expression be homologous,and patients with clinical stage,tumor size,whether the transfers are closely related,in feces of MTHFR,CK20,MMP-11,CD24,and COX-2mRNA for testing,can be effective in the diagnosis of colorectal cancer occurrence and development situation,joint test the application efficiency is higher.
