糖尿病患者健康强化管理的效果分析

Effect Analysis of Health Intensive Management for Diabetes Patients

目的对基于家庭医生团队的2型糖尿病患者健康强化管理干预进行效果评价。方法选取在某社区卫生服务中心就诊并签约的2型糖尿病患者作为研究对象,随机分为观察组和对照组,并进行为期1年的干预。其中,观察组给予强化管理,对照组进行常规管理。干预后,比较两组患者的相关实验室指标水平以及相关知识、行为的得分情况。结果干预后,观察组患者的BMI、空腹血糖、餐后2h血糖、糖化血红蛋白水平显著低于对照组,差异有统计学意义(P<0.01);观察组患者的健康知识、合理膳食、运动锻炼、就医行为、服药依从、家庭参与等方面的得分显著高于对照组,差异有统计学意义(P<0.01)。结论基于家庭医生团队的2型糖尿病患者健康强化管理干预有更好的效果,值得推广。

Objective To evaluate the effect of intensive health management intervention for type 2 diabetes patients based on family doctor team.Methods The patients with type 2 diabetes who seek medical in a community health care center and signed a contract were randomly divided into an observation group and a control group for one-year intervention.The observation group was given intensive management,and the control group was given routine management.After intervention,the levels of relevant laboratory indicators and scores of related knowledge and behavior between the two groups of patients were compared.Results After the intervention,the levels of BMI,fasting blood sugar,2-hours post prandial glucose,and Glycated hemoglobin in the observation group were significantly lower than those in the control group(P<0.01);the observation group had significantly higher scores than the control group in terms of health knowledge,reasonable diet,exercise,medical behavior,medication compliance,and family participation,with statistical significance(P<0.01).Conclusion The intensive health management intervention for type 2 diabetes patients based on the family doctor team has a better effect and is worth promoting.
