链式护理在社区存在压力性损伤风险人群中的应用效果评价

Evaluation of the Effect of Chain Management on People at Risk of Pressure Ulcers in Communities

目的探讨链式护理在社区存在压力性损伤风险患者中的应用效果。方法选取2021年4-5月在上海市徐汇区龙华街道社区卫生服务中心4个社区卫生站中存在压力性损伤风险的患者及其照顾者作为研究对象,按照居住小区将其分为对照组(n=76)和观察组(n=77)。对照组给予常规健康随访;观察组给予链式护理。干预3个月后,比较两组患者压力性损伤发生率及患者照顾者压力性损伤认知水平、照顾行为和照顾负担情况。结果干预3个月后,观察组患者压力性损伤发生率明显低于对照组(P<0.05);且观察组照顾者压力性损伤认知水平、照顾行为和负担情况得分均优于对照组(P<0.01)。结论链式护理有助于改善社区存在压力性损伤风险患者照顾者的压力性损伤认知水平,提升其照顾能力、减轻照顾负担,进而降低高风险患者压力性损伤的发生率。

Objective To explore the effect of chain management on people at risk of pressure ulcers in communities.Methods Patients at risk of pressure ulcers and their caregivers were selected from four community health stations of the Community Health Service Center of Longhua Road Subdistrict,Xuhui District,Shanghai from April to May 2021.They were divided into the control group(n=76)and the observation group(n=77)according to their residential areas.The control group was given routine health follow-up,while the observation group was given chain management.After 3 months of intervention,the incidence of pressure ulcers in patients and the cognitive level of pressure ulcers,care behavior and care burden in caregivers were compared between the two groups.Results After 3 months of intervention,the incidence of pressure ulcers in the observation group was significantly lower than that in the control group(P<0.05);caregivers in the observation group were superior to those in the control group in terms of the scores of cognitive level of pressure ulcers,care behavior and care burden(P<0.01).Conclusion Chain management helps to improve the cognitive level of pressure ulcers in caregivers of patients at risk of pressure ulcers in communities,enhance their care ability and reduce the burden of care,thereby reducing the incidence of pressure ulcers in high⁃risk patients.
