二次剖宫产腹壁竖切口改良式心型缝合的效果分析

目的探讨二次剖宫产腹壁竖切口改良式心型缝合的效果。方法选取2020年3月至2021年9月本院收取的120例二次剖宫产产妇为观察对象,按照缝合方法不同随机分为对照组和观察组,每组各60例。观察组采用腹壁竖切口改良式心型缝合,对照组采用传统U型缝合,术后连续随访1、3、6、12、18个月,对比两组患者术后切口愈合的各项指标,且将观察组与其前次剖宫产术后愈合进行对比。结果采用心形缝合技术的产妇在术后脂肪液化、切口渗液、愈合等级、温哥华瘢痕量表(VSS)评分、术后疤痕宽度、切口疼痛等指标方面与传统技术相比有明显改善(P<0.05)。结论二次剖宫产腹壁竖切口改良式心型缝合显著减轻脂肪液化、术后发热、切口疼痛等症状,显著减少术后瘢痕宽度、瘢痕增生,具有广泛推广意义。

Objective To explore the effect of heart-shaped cosmetic suture on vertical incision of abdominal wall undergoing Second Cesarean Section.Methods 120 patients of second cesarean section in our hospital from March 2020 to Sep 2021 were selected as the research objects.Depending on the suture method,they were randomly divided into the observation group and the control group with 60 patients in each group.Routine cosmetic suture was used in the control group and heart-shaped cosmetic suture was used in the observation group.The patients were followed up for 1.3.6.12.18 months,the indexes of postoperative incision healing were compared between the two groups,and the observation group was compared with the postoperative healing of previous cesarean section.Results Healing grade of the incision in the observation group were improved than those in the control group,with statistical significance(P<0.05).And the Fat liquefaction,wound extravasating,VSS score,postoperative scar width and postoperative incision pain of the observation group were lower than those of the control group,with statistical significance(P<0.05).Conclusion The application of heart-shaped cosmetic suture in the second cesarean section can significantly reduce fat liquefaction,wound extravasating,postoperative incision pain,scar width and scar hyperplasia.It is of great significance to popularize this improved suture method.
