动态增强磁共振成像联合糖类抗原CA15-3和血小板衍生生长因子对乳腺癌的诊断价值 被引量:1

Diagnostic value of dynamic enhanced magnetic resonance imaging combined with glycan antigen CA15-3 and platelet-derived growth factor in breast cancer

摘要 目的分析动态增强磁共振成像(DCE-MRI)联合糖类抗原CA15-3(CA15-3)和血小板衍生生长因子(PDGF)对乳腺癌的诊断价值,并分析其与乳腺癌发生的相关性。方法选取2021年12月—2022年12月在台州市中心医院治疗的48例乳腺癌患者为乳腺癌组,另选取同期在该院进行健康检查的48例健康女性为健康组。比较两组女性的DCE-MRI影像表现、CA15-3水平及PDGF水平;分析DCE-MRI、CA15-3、PDGF与乳腺癌发生的相关性;分析DCE-MRI、CA15-3、PDGF三者单独检测和联合检测对乳腺癌的诊断价值。结果乳腺癌组患者的各DCE-MRI影像表现均明显高于健康组女性,差异均有统计学意义(均P<0.05)。乳腺癌组患者的CA15-3、PDGF水平[(43.65±10.31)U/ml、(601.21±88.63)pg/ml]均显著高于健康组[(16.99±3.21)U/ml、(185.01±98.26)pg/ml],差异均有统计学意义(t=17.105、21.791,均P<0.05)。各DCEMRI参数(病灶短径、长径、边缘毛刺、ADC值及TIC分型)、CA15-3及PDGF表达水平均与乳腺癌发生呈显著正相关关系(r=0.564、0.661、0.618、0.631、0.603、0.596及0.616,均P<0.05)。ROC曲线显示,DCE-MRI联合CA15-3、PDGF诊断乳腺癌的曲线下面积为0.825,灵敏度为81.61%,特异度为83.27%,曲线下面积明显高于任意一项单独检测,具有良好的诊断效能。结论DCE-MRI参数、CA15-3、PDGF表达水平均与乳腺癌发生呈正相关关系,且DCE-MRI联合CA15-3、PDGF诊断乳腺癌具有突出价值,可显著提高乳腺癌诊断准确性,临床应用价值较高。 Objective To analyze the diagnostic value of Dynamic Enhanced Magnetic Resonance Imaging(DCE-MRI)combined with glycan antigen CA15-3(CA15-3)and Platelet Derived Growth Factor(PDGF)in the diagnosis of breast cancer and to analyze its correlation with the occurrence of breast cancer.Methods Forty-eight breast cancer patients treated in Taizhou Central Hospital from December 2021 to December 2022 were selected as the breast cancer group,and 48 healthy women who underwent health checkups in the hospital during the same period were selected as the health group.DCE-MRI imaging manifestations,CA15-3 levels and PDGF levels were compared between the two groups of women;To analyze the correlation between DCE-MRI,CA15-3,PDGF and breast carcinogenesis;To analyze the diagnostic value of DCE-MRI,CA15-3,and PDGF alone and in combination for breast cancer.Results The various DCE-MRI imaging manifestations of the patients in the breast cancer group were significantly higher than those of the women in the healthy group,and the differences were statistically significant(all P<0.05).The levels of CA15-3 and PDGF in patients in the breast cancer group[(43.65±10.31)U/ml,(601.21±88.63)pg/ml]were significantly higher than those in the healthy group[(16.99±3.21)U/ml,(185.01±98.26)pg/ml],and the differences were statistically significant(t=17.105,21.791,all P<0.05).Each DCE-MRI parameter(lesion short diameter,long diameter,edge burr,ADC value and TIC typing),CA15-3 and PDGF expression levels were significantly and positively correlated with breast carcinogenesis(r=0.564,0.661,0.618,0.631,0.603,0.596 and 0.616,all P<0.05).The ROC curve showed that the area under the curve of DCE-MRI combined with CA15-3 and PDGF in the diagnosis of breast cancer was 0.825,the sensitivity was 81.61%,the specificity was 83.27%,and the area under the curve was significantly higher than that of either individual test,which had good diagnostic efficacy.Conclusion DCE-MRI parameters,CA15-3,and PDGF expression levels are positively correlated with breast cancer occurrence,and DCE-MRI combined with CA15-3 and PDGF has outstanding value in the diagnosis of breast cancer,which can significantly improve the diagnostic accuracy of breast cancer,and has high clinical application value.
