摘要
目的探讨分析老年阿尔茨海默病(AD)患者、非长寿健康体检老人和长寿健康体检老人外周血血清同型半胱氨酸(Hcy)、尿酸(UA)和血脂的水平差异及意义。方法选择65岁以上阿尔茨海默病患者60例作为AD组、65~89岁非长寿健康体检老人60例作为非长寿组、90岁以上长寿健康体检老人60例作为长寿组。测定血清Hcy、尿素氮(BUN)、肌酐(Cr)、UA、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平。结果除UA、TC和HDL-C外,BUN、Cr、TG、HDL-C、LDL-C和Hcy水平差异均具有统计学意义(P<0.05或P<0.001)。与非长寿组比较,AD组Cr、LDL-C、Hcy水平及长寿组BUN、TG、LDL-C、Hcy水平更高(P<0.05或P<0.001);与长寿组相比,AD组Cr水平更高,BUN、TG水平更低,差异均具有统计学意义(P<0.05)。二元Logistic回归分析结果显示,经校正性别与年龄后,模型1(单独纳入非长寿组)TG、LDL-C、Hcy水平升高以及模型2(非长寿组合并长寿组)Cr、LDL-C、Hcy水平增高均显著增加了AD的发病风险。结论老年人群中血清Cr、TG、LDL-C和Hcy水平升高可显著增加AD的发病风险,提示控制上述生物标志物水平对于老年人群预防AD具有重要意义。
Objective To investigate the difference and significance of serum homocysteine(Hcy),uric acid(UA)and lipid levels in the peripheral blood of geriatric patients with Alzheimer's disease(AD),non-longevity healthy elderly and longevity healthy elderly.Methods Sixty AD patients over 60 years old were selected as AD group,sixty non-longevity healthy elderly aged 65~89 years old as non-longevity group,and sixty longevity healthy elderly aged over 90 years old as longevity group.The serum Hcy,urea nitrogen(BUN),creatinine(Cr),UA,total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)levels were determined.Results There were significant differences in BUN,Cr,TG,HDL-C,LDL-C and Hcy levels except UA,TC and HDL-C(P<0.05 or P<0.001).Serum Cr,LDL-C and Hcy levels in AD group and serum BUN,TG,LDL-C and Hcy levels in longevity group were higher compared to the non-longevity group(P<0.05 or P<0.001).Compared with longevity group,serum Cr levels in AD group were higher,and serum BUN and TG levels were lower,the differences were statistically significant(P<0.05).Binary Logistic regression analysis showed that the elevated levels of TG,LDL-C and Hcy in Model 1(including the non-longevity group alone)and the raised levels of Cr,LDL-C and Hcy in Model 2(the non-longevity group combined with the longevity group)significantly increased the risk of AD,after adjusting for gender and age.Conclusion Elevated concentrations of Cr,TG,LDL-C and Hcy in the geriatric population can remarkably increase the risk of AD,suggesting that controlling levels of these biomarkers is of great significance for the prevention of AD in these population.
