传统“L”型切口与改良切口微型钛板内固定治疗跟骨骨折的疗效对比分析

Comparative analysis of therapeutic effect of titanium plate internal fixation with modified incision in tarsal sinus on calcaneal fracture

目的探讨跟骨骨折采用跗骨窦改良切口微型钛板内固定治疗与“L”型切口可塑性加压钢板内固定的疗效对比。方法选取2020年4月至2022年4月在广西柳钢医疗有限公司医院就诊的96例跟骨骨折患者作为研究对象,按照随机数表法分为对照组与研究组,各48例,对照组采用“L”型切口可塑性加压钢板内固定,研究组采用跗骨窦改良切口微型钛板内固定。对两组患者术后的治疗效果、并发症(关节畸形、关节疼痛、关节活动障碍)发生率以及骨折愈合情况进行对比分析。结果研究组的治疗有效率为87.50%,显著高于对照组的66.67%(P<0.05)。研究组的并发症发生率为6.25%,显著低于对照组的25.00%(P<0.05)。研究组的骨折愈合时间为(61.57±2.51)d,显著短于对照组的(72.52±2.47)d。研究组的骨折愈合良好率为89.58%,显著高于对照组的64.58%,差异有统计学意义(P<0.05)。治疗前,对两组患者均进行负性情绪评分,两组患者评分水平差异无统计学意义(P>0.05);治疗后,两组患者的负性情绪均有所改善,且研究组的改善程度显著高于对照组(P<0.05)。结论采用跗骨窦改良切口微型钛板内固定治疗跟骨骨折的治疗效果优于“L”型切口可塑性加压钢板,并发症的发生率亦较低,可以缩短患者的骨折愈合时间,改善患者的负性情绪,进而提高患者的生活质量。

Objective To compare the therapeutic effects of modified tarsal sinus incision with mini titanium plate internal fixation and"L"shaped incision with plastic compression plate internal fixation for calcaneal fractures.Methods 96 patients with calcaneal fractures who were treated at the hospital of Guangxi Liugang Medical Co.,Ltd.from April 2020 to April 2022 were selected as the study subjects.They were randomly divided into a control group and a study group,with 48 cases in each group.The control group was treated with"L"shaped plastic compression plate internal fixation,while the study group was treated with modified tarsal sinus incision micro titanium plate internal fixation.Compare and analyze the postoperative treatment effectiveness,incidence of complications(joint deformities,joint pain,joint mobility disorders),and fracture healing of two groups of patients.Results The treatment effectiveness rate of the research group was 87.50%,higher than 66.67%of the control group(P<0.05).The incidence of complications in the study group was 6.25%,lower than the 25.00%in the control group(P<0.05).The fracture healing time of the study group was(61.57±2.51)days,significantly shorter than the control group's(72.52±2.47)days.The good rate of fracture healing in the study group was 89.58%,significantly higher than the 64.58%in the control group,with a statistically significant difference(P<0.05).Before treatment,both groups of patients were scored for negative emotions,and there was no statistically significant difference in the scores between the two groups(P>0.05);After treatment,the negative emotions of both groups of patients improved,and the degree of improvement in the study group was significantly higher than that in the control group(P<0.05).Conclusion The treatment effect of using modified tarsal sinus incision micro titanium plate internal fixation for calcaneal fractures is better than that of"L"shaped incision plastic compression plate,and the incidence of complications is also lower.It can shorten the fracture healing time of patients,improve their negative emotions,and thereby improve their quality of life.
