早产儿呼吸窘迫综合征早期口服维生素AD对重度支气管肺发育不良发生情况及炎症因子的影响

Effect of early oral vitamin AD on the occurrence of severe bron-chopulmonary dysplasia and inflammatory factors in premature infants with respiratory distress syndrome

摘要 目的探讨早产儿呼吸窘迫综合征早期口服维生素AD对重度支气管肺发育不良(BPD)发生情况及炎症因子的影响。方法选取2017年1月至2022年10月东南大学医学院附属江阴医院新生儿监护病房收治的90例呼吸窘迫综合征早产儿,根据住院号奇偶数分为对照组和早期治疗组,每组各45例。对照组在出生2周后给予维生素AD口服,早期治疗组在出生后进行胃肠内喂养后即给予维生素AD口服。比较两组呼吸支持时间、并发症发生率及炎症因子水平。结果早期治疗组呼吸支持时间短于对照组,重度BPD发生率低于对照组(P<0.05)。两组晚发型败血症、坏死性小肠结肠炎发生率比较,差异无统计学意义(P>0.05)。早期治疗组白细胞介素-6低于对照组(P<0.05)。两组白细胞介素-1β、白细胞介素-8及肿瘤坏死因子-α比较,差异无统计学意义(P>0.05)。结论早产儿呼吸窘迫综合征早期口服维生素AD可缩短呼吸支持时间,降低重度BPD发生率及IL-6水平,值得推广应用。 Objective To investigate the effect of early oral vitamin AD on the occur rence of severe bronchopulmonary dysplasia(BPD)and inflammatory factors in premature infants with respiratory distress syndrome.Methods A total of 90 premature infants with respiratory distress syndrome admitted to the Neonatal care unit of Jiangyin Hospital Affiliated to Southeast University School of Medicine from January 2017 to October 2022 were selected and divided into the control group and the early treatment group according to the odd and even number of hospitalization,with 45 cases in each group.The control group was given vitamin AD orally 2 weeks after birth,and the early treatment group was given vitamin AD orally after gastrointestinal feeding after birth.Respiratory support time,incidence of complications and inflammatory factor levels were compared between the two groups.Results The respiratory support time of the early treatment group was shorter than that of the control group,and the incidence of severe BPD was lower than that of the control group(P<0.05).There was no significant difference in the incidence of late onset sepsis and necrotizing enterocolitis between the two groups(P>0.05).Interleukin-6 in the early treatment group was lower than that in the control group(P<0.05).There were no significant differences in interleukin-1β,interleukin-8 and tumor necrosis factor-αbetween the two groups(P>0.05).Conclusion Early oral vitamin AD in premature infants with respiratory distress syndrome can shorten the respiratory support time,red uce the incidence of severe BP D and IL-6 level,which is worthy of popularization and application.
