老年COPD患者并发骨质疏松与LRP5基因多态性的相关性研究

Study on the relationship between low density lipoprotein receptor associated protein 5 and susceptibility to osteoporosis in elderly patients with chronic obstructive pulmonary disease

目的探讨老年慢性阻塞性肺疾病(COPD)患者并发骨质疏松与低密度脂蛋白受体相关蛋白5(LRP5)的相关性。方法回顾性分析2017年3月至2020年3月宜宾市第二人民医院收治的380例老年COPD患者临床资料。根据是否伴有骨质疏松将患者分为观察组(伴骨质疏松,n=208)和对照1组(无骨质疏松,n=172)。另收集同期老年非COPD骨质疏松患者作为对照2组(单纯骨质疏松的非COPD患者,n=84),老年体检健康者作为对照3组(n=84)。对4组的血液DNA进行遗传分析,分析患者LRP5的tagSNP基因型。采用Logistic回归分析患者在不同模式下SNP的OR值,评价COPD患者在发病时LRP5与骨骼并发症的相关性。结果根据隐性、显性、共显性等遗传模式下SNP的检验值,rs671494、rs312016、rs638051及rs901823为符合条件的SNP因素。Logistic回归分析结果显示,携带C/C基因型的COPD患者及LRP5基因的rs901823、rs671494基因型与患者罹患骨质疏松的相关程度最高,差异有统计学意义(P<0.05)。结论LRP5基因中的rs901823、rs671494与COPD患者并发骨质疏松相关,有助于判断骨质疏松发生风险。

Objective To study the relationship between low density lipoprotein receptor associated protein 5(LRP5)and susceptibility to osteoporosis in elderly patients with chronic obstructive pulmonary disease(COPD).Methods Retrospective analysis of the clinical data of 380 elderly patients with COPD admitted to Yibin Second People's Hospital from March 2017 to March 2020.The patients were divided into observation group(with osteoporosis,n=208)and control group 1(without osteoporosis,n=172)according to whether they were accompanied by osteoporosis.In addition,patients with non COPD osteoporosis in the same period were collected as control group 2(non COPD patients with simple osteoporosis,n=84),and healthy people were taken as control group 3(n=84).Genetic analysis was performed on the blood DNA of the four groups,and the tagSNP genotype of LRP5 was analyzed.The OR value of SNP of patients in different modes was analyzed by Logistic regression,and the correlation between LRP5 and bone complications in patients with COPD was evaluated.Results According to the SNP test values under recessive,dominant,Codominance and other genetic models,rs671494,rs312016,rs638051 and rs901823 are eligible SNP factors.The results of logistic regression analysis showed that COPD patients carrying the C/C genotype and the rs901823 and rs671494 genotypes carrying the LRP5 gene had the highest correlation with osteoporosis,the difference was statistically significant(P0.05).Conclusion The rs901823,rs671494 in LRP5 gene is associated with osteoporosis in elderly COPD patients,which is helpful to judge the risk of osteoporosis.
