基于胃微生态探讨胃癌前病变的中医药防治机制

Exploration of the Prevention and Treatment Mechanism of Pregastric Cancer with Traditional Chinese Medicine Based on Gastric Microecology

胃癌是我国常见的恶性肿瘤之一,其发病率和病死率极高,医疗资源消耗巨大,防控形势日益严峻。胃癌前病变是胃癌发生发展的重要环节,具有一定的可逆性,是胃癌早期诊治和预防的关键。大多数胃癌确诊患者处于进展期或晚期,其防治效果尚不理想。因此,寻求积极有效的防治策略极其重要。随着人类元基因组技术的开展和运用,发现胃微生态在胃癌的发生、发展和防治中扮演着重要的角色。胃微生态由肠道菌群、肠道细胞、黏膜化学屏障、免疫屏障和肠内神经等构成,参与机体的物质代谢、免疫调节等方面,已有研究表明,胃微生态失调对胃癌及癌前病变存在潜在的关系,因此,恢复胃微生态平衡极为重要。文章以胃微生态为切入点,阐述了胃微环境失衡与胃癌前病变的相关性,通过调节胃内环境、缓解黏膜炎症和免疫应答等方面,系统总结中医药干预胃癌前病变的发病机制,为中医药防治胃癌前病变提供新的方向。

Gastric cancer is one of the most common malignant tumors in China,which has a very high incidence and fatality rate.Precancerous lesion is an important link in the occurrence and development of gastric cancer.It has certain reversibility and is the key to the early diagnosis,treatment and prevention of gastric cancer.Most of the patients diagnosed with gastric cancer are in the advanced or advanced stage,and the prevention and treatment effect is not ideal.Therefore,it is extremely important to seek positive and effective prevention and control strategies.With the development and application of human metagenomic technology,gastric microecology has been found to play an important role in the occurrence,development and prevention of gastric cancer.Gastric microecology is composed of intestinal flora,intestinal cells,mucosal chemical barrier,immune barrier and intestinal nerves,which are involved in body metabolism and immune regulation,etc.Previous studies have shown that the imbalance of gastric microecology has a potential relationship with gastric cancer and precancerous lesions.Therefore,it is extremely important to restore the balance of gastric microecology.In this paper,the relationship between the imbalance of the gastric microenvironment and the precancerous lesions was expounded,and the pathogenesis of the intervention of traditional Chinese medicine in the precancerous lesions was systematically summarized by regulating the gastric environment,alleviating mucosal inflammation and immune response,providing a new direction for the prevention and treatment of precancerous lesions by traditional Chinese medicine.
