基于络病理论探析消化性溃疡活动期病因病机

Analysis of Etiology and Pathogenesis of Peptic Ulcer in Active Period Based on Collateral Disease Theory

在临床上,消化性溃疡属于“胃疡”“胃痈”“胃脘痛”等一类的消化道疾患。其病位在胃腑,与脾与肝(胆)相关,多认为消化性溃疡的病因与“毒热”有关。疾病活动期病因为外邪伤中,或情志犯胃,胃络气机壅阻,日久累积,而化毒热,致使肉腐血败,疾病发展符合中医络病理论中“久病入络”。故可从中医络病理论出发,探讨消化性溃疡活动期的病因病机与“毒热”理论的关系,进一步明确和论证“毒热”的病因观,“病因毒起,热因毒化”病机观。从络病理论角度阐明消化性溃疡病机演变规律。

In clinical practice,peptic ulcer belongs to a category of digestive tract diseases such as“gastric ulcer”,“gastric carbuncle”and“epigastric pain”.The disease is located in the stomach and is related to spleen and liver(gallbladder).It is often believed that the cause of peptic ulcer is related to“toxic heat”.During the active phase of the disease,due to external pathogenic factors causing damage or emotional invasion of the stomach,the Qi movement of the stomach meridians is obstructed and accumulated over time,leading to the transformation of toxic heat,resulting in putrefaction of blood and muscle.The development of the disease is in line with the traditional Chinese medicine theory of“prolonged illness entering the meridians”.As a result,starting from the theory of collateral diseases in traditional Chinese medicine,it explored the relationship between the etiology and pathogenesis of active peptic ulcer and the theory of“toxic heat”,further clarified and demonstrated the etiology of“toxic heat”,and the pathogenesis of“disease caused by toxin and heat caused by toxin”,elucidating the evolution law of peptic ulcer pathogenesis from the perspective of collateral disease theory.
