经皮肝穿刺引流术联合柴胡疏肝散治疗肝脓肿疗效观察

Curative Effect of Percutaneous Transhepatic Cholangial Drainage Combined with Chaihu Shugan Powder(柴胡疏肝散)on Liver Abscess

目的观察经皮肝穿刺引流术联合柴胡疏肝散治疗肝脓肿效果。方法随机数表法将本院2020年3月—2022年3月收治的60例肝脓肿患者分为观察组和对照组两组,每组各30例。对照组采取经皮肝穿刺引流术治疗,观察组在对照组基础上联合柴胡疏肝散治疗。治疗1个月后,比较两组患者治疗效果及并发症发生情况,同时比较治疗前后两组患者炎症水平、肝功能指标、免疫功能指标。结果治疗后,观察组患者治疗总有效率为96.67%(29/30)明显高于对照组的73.33%(22/30),差异具有统计学意义(P<0.05);观察组患者白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞计数(WBC)水平均明显低于对照组,差异具有统计学意义(P<0.05);观察组患者谷丙转移酶(ALT)、谷草转氨酶(AST)、谷氨酰转肽酶(GGT)水平明显低于对照组,血清白蛋白(ALB)水平明显高于对照组,差异具有统计学意义(P<0.05);观察组患者CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)水平明显高于对照组,CD_(8)^(+)水平明显低于对照组,差异具有统计学意义(P<0.05);观察组患者并发症发生率为6.67%(2/30)明显低于对照组的30.00%(9/30),差异具有统计学意义(P<0.05)。结论经皮肝穿刺引流术联合柴胡疏肝散能有效促进肝脓肿患者恢复,降低机体炎症水平,改善患者肝功能,提高其机体免疫能力,并发症较少。

Objective To observe the curative effect of percutaneous transhepatic cholangial drainage(PTCD)combined with Chaihu Shugan Powder(柴胡疏肝散)on liver abscess.Methods According to random number table method,60 patients with liver abscess admitted to the hospital were divided into observation group and control group between March 2020 and March 2022,30 cases in each group.The control group was treated with PTCD,while the observation group was additionally treated with Chaihu Shugan Powder.After 1 month of treatment,the curative effect and the occurrence of complications,inflammation level,liver function and immune function indexes before and after treatment were compared between the two groups.Results After treatment,the total response rate of treatment in the observation group was significantly higher than that in the control group[96.67%(29/30)vs 73.33%(22/30)](P<0.05).The levels of interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and white blood cell count(WBC)in the observation group were significantly lower than those in the control group(P<0.05).The levels of alanine transferase(ALT),aspartate aminotransferase(AST)and glutamyl transpeptidase(GGT)in the observation group were significantly lower than those in the control group,while the level of serum albumin(ALB)was significantly higher than that in the control group(P<0.05).The levels of CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+)in the observation group were significantly higher than those in the control group,while the levels of CD_(8)^(+)was significantly lower than that in the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group[6.67%(2/30)vs 30.00%(9/30)](P<0.05).Conclusion PTCD combined with Chaihu Shugan Powder can effectively promote the recovery of patients with liver abscess,reduce the inflammation level,improve the liver function and immunity,with relatively fewer complications.
