摘要
目的探讨新型冠状病毒感染患者中医证型与淋巴细胞计数及其年龄的相关性。方法选取上海市第七人民医院和武汉雷神山医院50名新型冠状病毒肺炎患者。收集患者中医四诊、分型、合并症相关的数据资料,对其进行辨证分型,并对不同中医证型患者的淋巴细胞计数及其年龄进行比较分析。结果(1)新型冠状病毒感染患者各证型所占比例由高到低依次为:湿毒郁肺证>寒湿阻肺证=肺脾气虚证=气阴两虚证>疫毒闭肺证>气营两燔证。(2)新型冠状病毒感染患者各证型患者年龄由大到小依次为:气营两燔证>疫毒闭肺证>湿毒郁肺证>气阴两虚证>肺脾气虚证>寒湿阻肺证。(3)新型冠状病毒感染患者中医各证型血常规比较:①新型冠状病毒感染患者各证型患者淋巴细胞百分比从高到低依次为:寒湿阻肺证>气阴两虚证>肺脾气虚证>湿毒郁肺证>疫毒闭肺证>气营两燔证。②新型冠状病毒感染患者各证型患者淋巴细胞绝对值从高到低依次为:寒湿阻肺证>湿毒郁肺证>肺脾气虚证>气阴两虚证>疫毒闭肺证>气营两燔证。③新型冠状病毒感染患者各证型患者白细胞计数从高到低依次为:湿毒郁肺证>气营两燔证>疫毒闭肺证>寒湿阻肺证>肺脾气虚证>气阴两虚证。④新型冠状病毒肺炎患者普通型与重型相比性别差异无明显统计学意义(P>0.05);年龄、住院天数、合并症比较有明显统计学差异(P<0.05)。(4)新型冠状病毒肺炎患者普通型与重型相比在红细胞(RBC)、血红蛋白(Hb)、红细胞压积(HCT)、红细胞分布宽度(RDW-CV)、淋巴细胞百分比(LYM%)、淋巴细胞绝对值(LYM#)差异有统计学意义(P<0.05)。结论本研究提示新型冠状病毒感染患者湿毒郁肺证最多,气营两燔证最少;气营两燔证患者年龄最大,寒湿阻肺证年龄最小;寒湿阻肺证患者淋巴细胞百分比最高,气营两燔证患者淋巴细胞百分比最低;新型冠状病毒肺炎普通型患者年龄小于重型;新型冠状病毒肺炎普通型患者淋巴细胞百分比高于重型。
Objective To explore the correlation between TCM syndromes and lymphocyte count and age in patients with corona virus disease(COVID-19).Methods Fifty patients with COVID-19 from Shanghai Seventh People's Hospital and Wuhan Leishenshan Hospital were selected.The data related to the TCM four diagnostic methods,classification and comorbidities of patients was collected,making the syndrome differentiation and comparing and analyzing the lymphocyte counts and ages of patients with different TCM syndromes.Results(1)The proportions of the various syndrome types of patients with COVID-19 in descending order were dampness stagnation in lung syndrome>cold-dampness obstructing lung syndrome=lung-spleen Qi deficiency syndrome=deficiency of Qi and Yin syndrome>epidemic Qi blocking lung syndrome>blazing of both Qi and Ying syndrome.(2)The ages of patients with COVID-19 in descending order were blazing of both Qi and Ying syndrome>epidemic Qi blocking lung syndrome>dampness stagnation in lung syndrome>deficiency of Qi and Yin syndrome>lung-spleen Qi deficiency syndrome>cold-dampness obstructing lung syndrome.(3)Comparison of blood routines of different TCM syndrome types in patients with COVID-19:①The percentages of lymphocytes in patients with COVID-19 with different syndrome types were as follows:cold-dampness obstructing lung syndrome>deficiency of Qi and Yin syndrome>lung-spleen Qi deficiency syndrome>dampness stagnation in lung syndrome>epidemic Qi blocking lung syndrome>blazing of both Qi and Ying syndrome.②The absolute value of lymphocytes in patients with COVID-19 of each syndrome type in descending order was cold-dampness obstructing lung syndrome>dampness stagnation in lung syndrome>lung-spleen Qi deficiency syndrome>deficiency of Qi and Yin syndrome>epidemic Qi blocking lung syndrome>blazing of both Qi and Ying syndrome.③The white blood cell counts of patients with COVID-19 from high to low were dampness stagnation in lung syndrome>blazing of both Qi and Ying syndrome>pidemic Qi blocking lung syndrome>cold-dampness obstructing lung syndrome>lung-spleen Qi deficiency syndrome>deficiency of Qi and Yin syndrome.There was no statistically significant difference in gender between patients with COVID-19 in common and severe cases(P>0.05).There were significant statistical differences in age,length of hospitalization and comorbidities(P<0.05).(4)Compared with those in common and severe cases of patients with COVID-19,the red blood cell(RBC),hemoglobin(Hb),hematocrit(HCT),red blood cell distribution width(RDW-CV),lymphocyte percentage(LYM%)and absolute lymphocyte value(LYM%)were statistically significant(P<0.05).Conclusion This study suggests that dampness stagnation in lung syndrome is mostly common for patients with COVID-19 and blazing of both Qi and Ying syndrome was the least.The patients with blazing of both Qi and Ying syndrome were the oldest,and those with cold-dampness obstructing lung syndrome were the youngest.The patients with cold-dampness obstructing lung syndrome had the highest lymphocyte percentage.The percentage of lymphocytes in patients with blazing of both Qi and Ying syndrome was the lowest and those with common type of COVID-19 were younger than those with severe cases.And the percentage of lymphocytes in patients with common type of COVID-19 was higher than that with severe type.
