三仁汤加减治疗对青光眼术后眼压控制的影响

Effects of Modified Sanren Decoction(三仁汤加减)on Intraocular Pressure Control after Glaucoma Surgery

目的探究三仁汤加减治疗对青光眼术后眼压控制的影响。方法将2018年1月—2021年1月接受手术治疗的80例青光眼患者纳入为研究对象,采用随机数字表法分为观察组40例(67眼)和对照组40例(70眼)。对照组给予美开朗滴眼液常规治疗,观察组在对照组基础上予以三仁汤加减治疗。比较两组治疗前及治疗3个月后视力、眼压、中医症状积分、视野指标[丢失方差(LV)、平均缺损(MD)、平均光敏感度(MS)]、视觉电生理指标(LP100潜伏期、AP100振幅)、视盘形态指标(平均视网膜神经纤维厚度、盘沿着面积)。结果治疗3个月后,观察组视力较治疗前显著提高(P<0.05),且显著高于对照组(P<0.05);两组眼压、中医症状积分、LV、MD、LP100潜伏期均较治疗前显著降低(P<0.05),且观察组显著低于对照组(P<0.05);两组MS、AP100振幅均较治疗前显著升高(P<0.05),且观察组显著高于对照组(P<0.05);观察组平均视网膜神经纤维厚度较治疗前显著增大(P<0.05),且显著大于对照组(P<0.05)。结论三仁汤加减治疗有利于青光眼患者术后眼压控制,可有效减轻患者证候表现,改善其视力、视野,同时有助于保护视神经功能,有较高临床应用价值。

Objective To explore the effects of Modified Sanren Decoction(三仁汤加减)on the intraocular pressure control after glaucoma surgery.Methods Eighty patients with glaucoma who underwent surgical treatment in the hospital between January 2018 and January 2021 were included as the study subjects,and they were divided into observation group(40 cases,67 eyes)and control group(40 cases,70 eyes)by the random number table method.The control group was given routine treatment with Mikelan eye drops,and the observation group was given Modified Sanren Decoction on the basis of the control group.The visual acuity,intraocular pressure,scores of TCM symptoms,visual field indicators[loss variance(LV),mean defect(MD),mean light sensitivity(MS)],visual electrophysiological indicators(LP100 latency,AP100 amplitude)and optic disc morphology indicators(average retinal nerve fiber thickness,rim area)were compared between the two groups before treatment and after 3 months of treatment.Results After 3 months of treatment,the visual acuity in the observation group was significantly improved compared with that before treatment(P<0.05),and was significantly better than that in the control group(P<0.05).The intraocular pressure,the scores of TCM symptoms,the values of LV,MD and LP100 latency were significantly reduced in the two groups compared to those before treatment(P<0.05),and the above indicators were significantly lower in the observation group than those in the control group(P<0.05).The values of MS and AP100 amplitude in the two groups were significantly higher than those before treatment(P<0.05),and the two indicators in the observation group were significantly higher compared with those in the control group(P<0.05).The average retinal nerve fiber thickness in the observation group was significantly thickened compared with that before treatment(P<0.05),and was significantly thicker than that in the control group(P<0.05).Conclusion Modified Sanren Decoction is beneficial to the control of postoperative intraocular pressure in patients with glaucoma,and it can effectively reduce the symptoms,improve the visual acuity and visual field,and help to protect the optic nerve function,with high clinical application value.
