老年发病的嗜酸性粒细胞性胃肠病1例并文献复习

A case report of eosinophilic gastrointestinal disease in the elderly with literature review

嗜酸性粒细胞性胃肠病(EGID)是一种消化道内嗜酸性粒细胞(EOS)浸润为主要特征的炎性疾病,病变可累及整个消化道全长或一部分,包括嗜酸性粒细胞性食管炎、胃炎、胃肠炎、小肠炎、结肠炎及直肠炎。该病是一种罕见的疾病,其中儿童及青壮年发病较高。由于EOS浸润程度及部位不同临床表现差异较大,可出现吞咽困难、腹痛、食欲下降、梗阻及胃食管反流相关症状、体重减轻、腹水、腹膜炎等多种不同的临床表现,确诊主要依赖病理活检,目前治疗方案主要为饮食调整的基础上予糖皮质激素治疗。本文回顾分析1例老年时期发病的病例,通过文献复习了解该病发病机制及诊疗方面进展。

Eosinophilic gastrointestinal disease(EGID)is an inflammatory disease mainly characterized by infiltration of eosinophils(EOS)in the digestive tract,which can affect the entire length or part of the digestive tract,including eosinophilic esophagitis,gastritis,gastroenteritis,enteritis,colitis,and proctitis.EGID is a rare disease,with higher incidence in children and young adults.As the significant differences in clinical manifestations due to the degrees and sites of eosinophil infiltration,there may be a variety of different clinical manifestations,such as dysphagia,abdominal pain,decreased appetite,obstruction,and gastroesophageal reflux related symptoms,weight loss,ascites,peritonitis,etc.The diagnosis mainly relies on pathological biopsy,and glucocorticoid therapy based on dietary adjustment is currently applied as the main treatment regimen.This article reviews and analyzes a case of disease onset in the elderly,and demonstrates the pathogenesis and progress in diagnosis and treatment of this disease by literatu re review.
