MSCT及3.0 T MRI在肾上腺肿瘤鉴别诊断中的应用价值

Application of MSCT and 3.0 T MRI in differential diagnosis of adrenal tumors

目的探讨多层螺旋CT(MSCT)及中高场强磁共振磁(3.0T MRI)在肾上腺肿瘤鉴别诊断中的应用价值。方法选取2019年4月至2020年4月河南大学第一附属医院收治的96例肾上腺肿瘤患者作为研究对象,均给予MSCT、3.0T MRI检查,以病理活检结果为金标准,统计两种检查方法对疾病的检出情况;并对比MSCT、3.0T MRI在肾上腺肿瘤中的诊断效能(诊断符合率、灵敏度、特异度、阳性预测值、阴性预测值、误诊率、漏诊率)。结果MSCT检查的诊断符合率、灵敏度、特异度、阳性预测值、阴性预测值分别为95.83%、94.83%、97.37%、98.21%、92.50%,均高于3.0T MRI检查的77.08%、74.14%、81.58%、86.00%、67.39%,差异有统计学意义(χ^(2)=14.413、9.469、5.029、5.648、8.150,均P<0.05);MSCT检查的误诊率、漏诊率分别为2.63%、5.17%,均低于3.0T MRI检查的18.42%、25.86%,差异有统计学意义(χ^(2)=5.029、9.469,均P<0.05)。结论采用MSCT检查肾上腺肿瘤,能有效鉴别诊断疾病,提高定性准确性,检查效果理想,能为临床诊治工作提供客观有力的数据。

【Objective】To investigate the use of multislice computed tomography(MSCT)and medium high field strength magnetic resonance(3.0 T MRI)in the differential diagnosis of adrenal tumors.【Methods】Ninety-six patients with adrenal tumors treated in our hospital between April 2019 and April 2020 were selected as the study subjects,all of whom were given MSCT,3.0 T MRI examination,and pathological biopsy results were used as the gold standard,and the detection status of the disease was statistically determined by the two examination methods.We also compared the diagnostic efficacy(diagnostic concordance rate,sensitivity,specificity,positive predictive value,negative predictive value,misdiagnosis rate,and missed diagnosis rate)of MSCT,3.0 T MRI in adrenal tumors.【Results】The diagnostic concordance rate,sensitivity,specificity,positive predictive value,and negative predictive value of MSCT test were 95.83%,94.83%,97.37%,98.21%,92.50%,respectively,which were higher than those of 3.0 T MRI test were 77.08%,74.14%,81.58%,86.00%,67.39%,and the differences were considered statistically significant(χ^(2)=14.413,9.469,5.029,5.648,8.150,all P<0.05).The misdiagnosis and missed rate of MSCT examination were 2.63%,5.17%,respectively,which were lower than those of 3.0 T MRI examination(18.42%,25.86%),and the differences were considered statistically significant(χ^(2)=5.029,9.469,all P<0.05).【Conclusion】Using MSCT to examine adrenal tumors can effectively differentiate diagnosis diseases and improve the qualitative accuracy,the examination is ideal,and can provide objective and powerful data for clinical diagnosis and treatment.
