生脉饮合清气化痰汤对机械通气患者下呼吸道多重耐药菌感染的影响

Effect of Shengmai Yin combined with QingqiHuatan Decoction on multi-drug resistant bacteria infection of the lower respiratory tract in mechanical ventilation patients

目的 观察生脉饮合清气化痰汤对机械通气患者下呼吸道多重耐药菌(MDRB)感染的临床疗效。方法 将60例机械通气MDRB感染患者按照随机数字表法分为2组,对照组30例予常规抗感染治疗,治疗组30例在对照组治疗基础上应用生脉饮合清气化痰汤治疗。2组均治疗14天。比较2组疗效;比较2组治疗前后炎症因子白细胞介素6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)水平;比较2组治疗前后痰液黏稠度;比较2组临床症状改善情况。结果 治疗组总有效率96.67%(29/30),对照组总有效率80.00%(24/30),治疗组疗效优于对照组(P<0.05)。2组治疗后IL-6、CRP、TNF-α水平均较本组治疗前降低(P<0.05),治疗后治疗组IL-6、CRP、TNF-α水平均低于对照组(P<0.05)。2组治疗后痰液黏稠度较本组治疗前改善(P<0.05),治疗后治疗组痰液黏稠度改善优于对照组(P<0.05)。治疗组体温恢复正常时间、咳嗽改善时间、白细胞计数水平恢复正常时间均短于对照组(P<0.05)。结论 生脉饮合清气化痰汤治疗机械通气MDRB感染患者,能通过降低患者炎症因子水平、改善痰液黏稠度来改善患者临床症状。

Objective To observe the therapeutic effect of Shengmai Yin combined with QingqiHuatan Decoction on multi-drug resistant bacteria(MDRB)infection of the lower respiratory tract in mechanical ventilation patients.Methods A total of 60 mechanical ventilation patients with MDRB infection were randomized to receive conventional anti-infection treatment(the control group,n=30)or Shengmai Yin combined with QingqiHuatan Decoction and conventional anti-infection treatment(the treatment group,n=30)for 14 days.The curative effects,the levels of interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α),the sputum viscosity,as well as the clinical symptoms were included as comparisons between groups before and after treatment.Results The total effective rate in the treatment group was better than that in the control group(96.67%[29/30]vs 80.00%[24/30],P<0.05).After treatment,the levels of IL-6,CRP and TNF-αin the both groups were significantly lower than those before treatment(P<0.05),and which were more pronounced in the treatment group than in the control group(P<0.05).The sputum viscosity in the both groups was better than that before treatment(P<0.05),and the improvement was more significant in the treatment group than in the control group(P<0.05).The time to normal body temperature,improvement in cough symptoms,and normal leukocyte level in the treatment group were significantly shorter than those in the control group(P<0.05).Conclusion For mechanical ventilation patients with MDRB infection,Shengmai Yin combined with QingqiHuatan Decoction can effectively improve the patients'curative symptoms of by reducing inflammatory factors level and improving sputum viscosity.
