血清AFP、CA19-9、CEA联合检验在原发性肝癌早期诊断中的应用价值

Application Value of Combined Detection of Serum AFP,CA19-9 and CEA in Early Diagnosis of Primary Liver Cancer

目的:分析血清甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)、癌胚抗原(CEA)在原发性肝癌早期诊断中的应用价值。方法:选取2019年1月—2022年12月高淳人民医院收治的疑似原发性肝癌患者42例作为肝癌组,选取同期健康体检者42例作为对照组。比较两组血清AFP、CA19-9、CEA水平。以病理检查结果为“金标准”,分析血清AFP、CA19-9、CEA单项及联合检验的诊断价值。结果:肝癌组AFP、CA19-9、CEA水平高于对照组,差异有统计学意义(P<0.001)。肝癌组患者经病理诊断阳性41例,阴性1例;经AFP诊断阳性30例,阴性12例;经CA19-9诊断阳性25例,阴性17例;经CEA诊断阳性19例,阴性23例。AFP、CA19-9、CEA单项检验及联合检验诊断原发性肝癌的受试者工作特征曲线下面积分别为0.787、0.701、0.689、0.821,三项联合检验的诊断价值最高。结论:血清AFP、CA19-9、CEA联合检验对原发性肝癌的诊断效能较高,对患者早期诊断及治疗具有重要临床价值。

Objective:To analyze the application value of serum alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9)and carcinoembryonic antigen(CEA)in the early diagnosis of primary liver cancer.Methods:A total of 42 patients with suspected primary liver cancer who were treated at Gaochun People's Hospital from January 2019 to December 2022 were selected as the liver cancer group.And 42 healthy individuals who underwent physical examination during the same period were selected as the control group.The serum AFP,CA19-9,and CEA levels of the two groups were compared.The results of pathological examination were used as the"gold standard"to analyze the diagnostic value of single detection and combined detection of serum AFP,CA19-9 and CEA.Results:The levels of AFP,CA19-9 and CEA in the liver cancer group were higher than those in the control group,and the differences were statistically significant(P<0.001).In the liver cancer group,41 patients were positive and 1 patient was negative by pathological diagnosis;30 patients were positive and 12 patients were negative by AFP diagnosis;25 patients were positive and 17 patients were negative by CA19-9 diagnosis;19 patients were positive and 23 patients was negative by CEA diagnosis.The areas under receiver operating characteristic curve of single detection and combined detection of serum AFP,CA19-9 and CEA for the diagnosis of primary liver cancer were 0.787,0.701,0.689 and 0.821,respectively,and the diagnostic value of the three-item combined detection was the highest.Conclusion:The combined detection of serum AFP,CA19-9 and CEA has a high diagnostic efficacy for primary liver cancer,and has important clinical value for early diagnosis and treatment of patients.
