彩色多普勒超声诊断乳腺良恶性肿块的临床价值

Clinical Value of Color Doppler Ultrasound in Diagnosis of Benign and Malignant Breast Mass

目的:探讨彩色多普勒超声诊断乳腺良恶性肿块的临床价值。方法:选取2018年3月—2021年3月东明县人民医院收治的乳腺肿块患者78例作为研究对象,患者均行乳腺钼靶X线检查与彩色多普勒超声检查,以病理检查结果为“金标准”,比较乳腺钼靶X线与彩色多普勒超声的诊断效能。结果:彩色多普勒超声诊断乳腺良恶性肿块的灵敏度、准确度高于乳腺钼靶X线,差异有统计学意义(P<0.05);两种检查方式的特异度比较,差异无统计学意义(P>0.05);Kappa检验显示:彩色多普勒超声检查与病理检查结果的一致性良好(Kappa=0.901,P<0.001);乳腺钼靶X线检查与病理检查结果的一致性不佳(Kappa=0.284,P=0.004)。彩色多普勒超声检查诊断乳腺肿块类型的准确度高于乳腺钼靶X线检查,差异有统计学意义(P<0.05)。结论:彩色多普勒超声能够准确鉴别乳腺肿块组织的病理类型,具有较高的诊断价值,且与病理检查结果的一致性良好,可为临床诊断提供有力的参考依据。

Objective:To investigate the clinical value of color Doppler ultrasound in the diagnosis of benign and malignant breast masses.Methods:A total of 78 patients with breast masses treated in Dongming County People's Hospital from March 2018 to March 2021 were selected as the study subjects.All patients underwent breast molybdenum target X-ray and color Doppler ultrasound.The results of pathological examination were used as the gold standard.The diagnostic efficacy of breast molybdenum target X-ray and color Doppler ultrasound was compared.Results:The sensitivity and accuracy of color Doppler ultrasound in the diagnosis of benign and malignant breast masses were higher than those of breast molybdenum target X-ray,and the difference was statistically significant(P<0.05).There was no significant difference in the specificity of the two examination methods(P>0.05).Kappa test showed a good agreement between color Doppler ultrasound and pathological examination(Kappa=0.901,P<0.001),and a poor agreement between breast molybdenum target X-ray and pathological examination(Kappa=0.284,P=0.004).The accuracy of color Doppler ultrasound in the diagnosis of breast mass type was higher than that of breast molybdenum target X ray,and the difference was statistically significant(P<0.05).Conclusion:Color Doppler ultrasound can accurately identify the pathological types of breast masses,has high diagnostic value,and has good consistency with the results of pathological examination.It can provide a powerful reference for clinical diagnosis.
