循证护理在胸外科术后患者中的应用效果

Application Effect of Evidence-based Nursing in Patients After Thoracic Surgery

目的:探讨循证护理在胸外科术后患者中的应用效果。方法:选取2019年5月—2021年5月于南京医科大学附属南京医院拟行胸外科手术的患者60例作为研究对象,随机分为观察组与对照组,各30例。对照组实施常规护理,观察组实施循证护理。比较两组护理前后凝血功能指标[纤维蛋白原(FIB)、凝血酶时间(TT)、活化部分凝血活酶(APTT)]及下肢深静脉血栓(DVT)形成率。结果:护理前,两组FIB、APTT、TT比较,差异无统计学意义(P>0.05);护理后,两组FIB水平低于护理前,且观察组低于对照组,两组TT、APTT长于护理前,但观察组短于对照组,差异有统计学意义(P<0.05)。观察组DVT形成率低于对照组,差异有统计学意义(P=0.030)。结论:循证护理有助于改善胸外科术后患者的血液高凝状态,预防DVT形成。

Objective:To explore the application effect of evidence-based nursing in patients after thoracic surgery.Methods:A total of 60 patients who were candidates for thoracic surgery in Nanjing Hospital Affiliated to Nanjing Medical University from May 2019 to May 2021 were selected as the study subjects.They were randomly divided into the observation group and the control group,with 30 cases in each group.The control group implemented routine nursing,and the observation group implemented evidence-based nursing.The coagulation function indexes[fibrinogen(FIB),thrombin time(TT),activated partial thromboplastin time(APTT)]before and after nursing,as well as the rate of deep vein thrombosis(DVT)formation of the lower extremity were compared between the two groups.Results:Before nursing,there were no significant differences in FIB,APTT and TT between the two groups(P>0.05).After nursing,the FIB level in the two groups was lower than that before nursing,and the observation group was lower than the control group,and the TT and APTT in the two groups were longer than those before nursing,but the observation group was shorter than the control group,and the difference was statistically significant(P<0.05).The DVT formation rate in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.030).Conclusion:The evidence-based nursing help to improve the blood hypercoagulability and prevent the formation of DVT in thoracic surgery patients.
