罗库溴铵与顺苯磺酸阿曲库铵在老年腹腔镜手术中的比较

Comparison of rocuronium and atracurium cisbenzenesulfonate in laparoscopic surgery under general anesthesia in the elderly

目的:探讨罗库溴铵与顺苯磺酸阿曲库铵在老年全麻腹腔镜手术中的应用效果。方法:选取医院收治的80例老年腹腔镜手术患者作为研究对象,随机分为罗库溴铵组与对照组,每组40例。罗库溴铵组患者在全身麻醉中应用罗库溴铵作为肌松药物,对照组患者应用顺苯磺酸阿曲库铵作为肌松药物,对比两组患者麻醉起效时间、插管时间、呛咳反应以及插管成功率,对比两组患者麻醉诱导前、诱导中、麻醉诱导后的生命体征与呼吸功能指标,并对比两组患者的不良反应情况。结果:罗库溴铵组麻醉起效时间、插管时间、呛咳反应发生率明显低于对照组(P<0.05),麻醉诱导前、诱导中、麻醉诱导后对照组HR、MAP、DBP、SBP差异具有统计学意义(P<0.05),诱导中和诱导后罗库溴铵组与对照组患者HR、MAP、DBP、SBP差异具有统计学意义(P<0.05);诱导中到麻醉诱导后两组患者PETCO_(2)均升高,但罗库溴铵组低于对照组,对照组患者诱导中到麻醉诱导后SpO_(2)降低,罗库溴铵组高于对照组(P<0.05);罗库溴铵组不良反应发生率明显低于对照组(P<0.05)。结论:在老年腹腔镜手术全麻诱导中,采取罗库溴铵更能够提升患者麻醉效果与苏醒质量,稳定患者术中生命体征,降低手术对呼吸功能带来的影响,减少不良反应,值得临床推广。

Objective:To compare the application of rocuronium and atracurium cisbenzenesulfonate in laparoscopic surgery under general anesthesia in the elderly.Methods:Eighty elderly patients who underwent laparoscopic surgery in hospital were selected as research objects,and were divided into rocuronium bromide group and control group by random number grouping method,with 40 cases in each group.Patients in the rocuronium bromide group were treated with rocuronium bromide as a muscle relaxant drug in general anesthesia,while patients in the control group were treated with atracuronium cisbenesulfate as a muscle relaxant drug.The onset time of anesthesia,intubation time,cough response and intubation success rate of patients in the two groups were compared,and the vital signs and respiratory function indexes of patients in the two groups were compared before,during and after anesthesia induction.The adverse reactions of the two groups were compared.Results:The effective time of anesthesia,intubation time and incidence of choking reaction in rocuronium bromide group were significantly lower(P<0.05),while there were significant differences in HR,MAP,DBP and SBP in control group before,during and after anesthesia induction(P<0.05).There were significant differences in HR,MAP,DBP and SBP between rocuronium bromide group and control group during and after induction(P<0.05).PETCO_(2) was increased between the two groups from induction to anesthesia induction,but the rocuronium bromide group was lower,the SpO_(2) of the control group was lower from induction to anesthesia induction,and the rocuronium bromide group was higher(P<0.05).The incidence of adverse reactions in rocuronium bromide group was significantly lower(P<0.05).Conclusion:For the induction of general anesthesia in elderly laparoscopic surgery,rocuronium bromide can improve the anesthetic effect and recovery quality of patients,stabilize the vital signs of patients during surgery,reduce the impact of surgery on respiratory function and reduce adverse reactions,which is worthy of clinical promotion.
