血清SAA、CRP及PCT对胎膜早破并发宫内感染的预测价值

The predictive value of serum SAA,CRP and PCT in premature rupture of membranes complicated with intrauterine infection

目的:探讨与分析血清淀粉样蛋白A(SAA)、C反应蛋白(CRP)、降钙素原(PCT)对胎膜早破并发宫内感染的预测价值,以促进早期检出宫内感染。方法:收集医院收治的88例胎膜早破产妇作为研究对象,检测血清SAA、CRP、PCT含量,判断宫内感染发生情况并进行预测价值分析。结果:在88例产妇中,发生宫内感染20例(宫内感染组),宫内感染组产妇的血清SAA、CRP、PCT含量与非宫内感染组对比有显著升高(P<0.05)。在88例产妇中,Spearsman分析显示血清SAA、CRP、PCT含量与宫内感染存在正向相关性(P<0.05)。Logistic多因素分析显示血清SAA、CRP、PCT含量都为导致宫内感染发生的主要独立危险因素(P<0.05)。受试者工作特征(ROC)曲线分析显示血清SAA、CRP、PCT含量对胎膜早破并发宫内感染预测的曲线下面积分别为0.730、0.751、0.714,联合检测的曲线下面积为0.872。结论:胎膜早破并发宫内感染产妇多伴随有血清SAA、CRP、PCT的高表达,血清SAA、CRP、PCT对胎膜早破并发宫内感染都有很好的预测价值,联合检测的预测价值更优。

Objective:To explore and analyze the predictive values of serum amyloid A(SAA),C-reactive protein(CRP)and procalcitonin(PCT)on premature rupture of membranes(PROM)complicated with intrauterine infection,so as to promote the early detection of intrauterine infection.Methods:88 cases of women with premature rupture of membranes(PROM)admitted to a certain hospital were collected as research subjects,and the contents of serum SAA,CRP and PCT were detected to determine the incidence of intrauterine infection and analyze the predictive value.Results:In the 88 cases of puerpera,there were 20 cases(intrauterine infection group)with intrauterine infection,The levels of serum SAA,CRP and PCT in the intrauterine infection group were significantly higher than those in the non-intrauterine infection group(P<0.05).Spearman analysis showed that the contents of serum SAA,CRP and PCT were positively correlated with intrauterine infection in 88 pregnant women(P<0.05).Logistic multivariate analysis showed that the contents of serum SAA,CRP and PCT were the main independent risk factors leading to intrauterine infection(P<0.05).The analysis of receiver operating characteristic(ROC)curve showed that the area under the curve of serum SAA,CRP,PCT content to predict premature rupture of membranes complicated with intrauterine infection were 0.730,0.751,0.714 respectively,and the area under the curve of combined detection were 0.872.Conclusion:The high expression of serum SAA,CRP and PCT in women with premature rupture of membranes(PROM)complicated with intrauterine infection is common.Serum SAA,CRP and PCT have good predictive value for PROM complicated with intrauterine infection,and the combined detection has better predictive value.
