鼓室负压及积液对畸变产物耳声发射引出影响的初步观察

摘要 目的探索环境压与鼓室峰压补偿下畸变产物耳声发射(DPOAE)检测在分泌性中耳炎患者中的引出情况。方法本研究为前瞻性研究设计,收集于2023年1月~2023年4月在北京同仁医院耳鼻咽喉科诊断为分泌性中耳炎且C型鼓室图行鼓膜切开术的患者,符合纳入与排除标准的病例25例,共30侧耳。均行环境压与鼓室峰值负压补偿下的DPOAE检测。患者年龄22岁~75岁,平均年龄53.97岁。男性15例,女性10例。右耳13例,左耳17例.均为鼓膜切开术确认是否存在鼓室积液者。结果30侧C型鼓室图患者25侧术中发现明显积液存在;5侧术中未见明显积液。30侧患耳中环境压DPOAE仅1侧引出,10侧鼓室峰值负压补偿下DPOAE在不同频率可以引出。结论对于鼓室图为C型的分泌性中耳炎患者,部分环境压DPOAE引不出者峰压补偿DPOAE在不同的频率可以引出。 Objectives To explore the detection of distortion product otoacoustic emission(DPOAE)in patients with otitis media with effusion under the ambient and compensated tympanic peak pressure.Methods This is a prospective study.Patients were enrolled in this study,who were diagnosed with secretory otitis media and underwent myringotomy in the Department of Otorhinolaryngology,Beijing Tongren Hospital from January 2023 to April 2023.A total of 25 cases(30 ears)met the inclusion and exclusion criteria.DPOAE was measured under the ambient and negative compensated tympanic peak pressure.The age of the patients ranged from 22 to 75 years(mean 53.97 years),including 15 males and 10 females.13 cases of right ear and 7 of left ear went through myringotomy to confirm the presence of tympanic effusion.Results 25 of 30 ears with type C tympanograms were found to have obvious effusion,while 5 with no obvious effusion during the operation.Only 1 ear could be elicited in the ambient pressure DPOAE test,and 10 ears could be elicited in the compensated tympanic peak pressure DPOAE test at different frequencies.Conclusion It could be elicited at different frequencies under compensated tympanic peak pressure for patients suffered from secretory otitis media with type C tympanogram,whose DPOAE cannot be measured under the ambient pressure.
