凝血4项(PT、APTT、TT、FIB)和D-二聚体在重症肺炎诊断中的应用效果分析

摘要 为探究凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)以及D-二聚体在重症肺炎诊断中的临床意义,采用回顾性分析法,随机选择2022年1月—2022年12月广西贺州市人民医院72例重症肺炎患者(观察组)和94例普通肺炎患者(对照组)作为研究对象,查看观察组重症肺炎病情是否进一步加重,进行分组分析。所有患者均进行凝血4项以及D-二聚体检查,比较检测结果。结果显示,观察组患者PT比对照组患者长(P<0.05),D-二聚体水平值明显比对照组患者高(P<0.05),观察组患者APTT、TT、FIB差异无统计学意义(P>0.05)。在观察组中,重症肺炎加重组患者PT、APTT比未加重组患者长(P<0.05),D-二聚体水平值明显比未加重组高(P<0.05),加重组TT、FIB与未加重组相比,差异无统计学意义(P>0.05)。研究表明,监测重症肺炎患者凝血4项指标以及D-二聚体水平可帮助医生了解患者凝血功能状态以及重症肺炎严重程度,有助于患者后续治疗,值得在临床上广泛使用。 To explore the clinical signifi cance of Prothrombin time(PT),activated partial thrombin time(APTT),thrombin time(TT),fi brinogen(FIB)and D-dimer in the diagnosis of severe pneumonia.Using retrospective analysis method,72 patients with severe pneumonia(observation group)and 94 patients with common pneumonia(control group)in The People’s Hospital of Hezhou from January 2022 to December 2022 were randomly selected as the research objects,to check whether the condition of severe pneumonia in the observation group was further aggravated,and to conduct grouping analysis.All patients were examined for 4 items of blood coagulation and D-dimer,and the test results were compared.The results showed that the PT of the patients in the observation group was longer than that of the patients in the control group(P<0.05),and the level of D-dimer was signifi cantly higher than that of the patients in the control group(P<0.05).There was no signifi cant diff erence in APTT,TT,FIB of the patients in the observation group(P>0.05).In the observation group,the PT and APTT of patients with severe pneumonia plus recombination were longer than those of patients without recombination(P<0.05),and the level of D-dimer was signifi cantly higher than that of patients without recombination(P<0.05).There was no signifi cant diff erence in TT and FIB between patients with severe pneumonia plus recombination and those without recombination(P>0.05).The research shows that monitoring the four coagulation indicators and the level of D-dimer in patients with severe pneumonia can help doctors understand the status of patients’coagulation function and the severity of severe pneumonia,which is helpful for patients’follow-up treatment and is worthy of extensive clinical use.
