经口腔前庭入路术式对甲状腺癌患者围术期指标及甲状旁腺功能的影响研究

The Effect of Oral Vestibular Approach on Perioperative Indicators and Parathyroid Function in Patients with Thyroid Cancer

为探讨经口腔前庭入路术式对甲状腺癌患者围术期指标及甲状旁腺功能的影响,选取2021年1月—2023年1月贵州省人民医院收治的80例甲状腺癌患者作为研究对象,按照收治顺序和患者对美观程度的需求将其平均分为对照组和实验组,每组各40例。对照组行传统开放手术,实验组行经口腔前庭入路术。比较两组患者围术期相关指标、不同时间段甲状旁腺功能、手术前后外周静脉血中C反应蛋白(CRP)、皮质醇(Cor)水平,并发症发生率和美观满意度。结果显示,手术后,实验组血钙与PTH浓度均高于对照组(P<0.05),CRP、Cor浓度均低于对照组(P<0.05),实验组患者并发症发生率低于对照组(P<0.05),美观满意度高于对照组(P<0.05)。研究发现,经口腔前庭入路术式可降低手术对患者甲状旁腺的损伤,减少手术对机体的创伤性刺激和并发症,可加快患者康复进程,值得推广应用。

Explore the effect of oral vestibular approach on perioperative indicators and parathyroid function in patients with thyroid cancer.80 thyroid cancer patients admitted to Guizhou Provincial People's Hospital from January 2021 to January 2023 were selected as the research subjects.They were evenly divided into two groups according to the order of admission and the patients'needs for aesthetic appearance,namely the control group and the experimental group,with 40 patients in each group.The control group underwent traditional open surgery,while the experimental group underwent transoral vestibular approach surgery.Compare perioperative related indicators,parathyroid function at different time periods,levels of C-reactive protein(CRP)and cortisol(Cor)in peripheral venous blood before and after surgery,incidence of complications,and aesthetic satisfaction between two groups of patients.The results showed that after surgery,the blood calcium and PTH concentrations in the experimental group were higher than those in the control group(P<0.05),while the CRP and Cor concentrations were lower than those in the control group(P<0.05).The incidence of complications in the experimental group was lower than that in the control group(P<0.05),and the satisfaction with aesthetics was higher in the experimental group than in the control group(P<0.05).Research has found that the oral vestibular approach can reduce the damage to the patient's parathyroid gland caused by surgery,reduce the traumatic stimulation and complications of surgery on the body,and accelerate the patient's recovery process.It is worthy of application.
