摘要
目的:分析增强CT联合血清DKK-1、可溶性淋巴细胞活化基因-3 (LAG-3)对甲状腺癌及其分期的诊断价值。方法:选择2018年1月—2020年1月民权县人民医院收治的83例甲状腺癌患者进行增强CT联合血清DKK-1、LAG-3检查。分析增强CT、血清DKK-1、LAG-3对甲状腺癌及其分期诊断准确率,分析其诊断价值。结果:增强CT诊断了83例甲状腺癌,诊断率为100.00%;其中63例准确为甲状腺癌,准确率为75.90%。误诊例数为20例,误诊率为24.10%。临床分期中增强CT诊断Ⅰ~Ⅱ期例数准确43例,准确率75.44%;Ⅲ~Ⅳ期诊断准确20例,准确率76.92%。增强CT诊断确诊患者血清DKK-1水平高于误诊患者,差异有统计学意义(t=59.261,P<0.05),LAG-3水平低于误诊患者,差异有统计学意义(t=36.214,P<0.05)。Ⅲ~Ⅳ期患者血清DKK-1水平高于Ⅰ~Ⅱ期,差异有统计学意义(t=41.269,P<0.05),LAG-3水平低于Ⅰ~Ⅱ期,差异有统计学意义(t=39.627,P<0.05)。增强CT联合血清DKK-1、LAG-3对甲状腺癌及其分期的诊断价值最高,差异有统计学意义(P<0.05)。结论:增强CT联合血清DKK-1、LAG-3能提升甲状腺癌及其分期准确性,协同提升诊断价值。
Objective:To analyze the diagnostic value of enhanced CT combined with serum DKK-1 and soluble lymphocyte activation gene-3(LAG-3) for thyroid cancer and its staging.Methods:83 patients with thyroid cancer treated in hospitals from January 2018 to January 2020 were selected and enhanced CT combined with serum DKK-1 and LAG-3 examinations were performed.The diagnostic accuracy and diagnostic value of enhanced CT,serum DKK-1 and LAG-3 on thyroid cancer and its staging were analyzed.Results:Enhanced CT diagnosed 83 cases of thyroid cancer,with a diagnostic rate of 100.00%.Among them,63 cases were accurate as thyroid cancer,with an accuracy rate of 75.90%,and the number of misdiagnosed cases was 20,with a misdiagnosis rate of 24.10%.The number of cases diagnosed by enhanced CT in clinical stage I-II was accurate in 43cases,with an accuracy rate of 75.44%,and the diagnosis of stage III-IV was accurate in 20 cases,with an accuracy rate of76.92%.Serum DKK-1 levels were higher in patients with confirmed diagnosis by enhanced CT than in patients with misdiagnosis,and the difference was statistically significant(t=59.261,P<0.05).LAG-3 levels were lower than those of misdiagnosed patients,and the difference was statistically significant(t=36.214,P<0.05).Serum DKK-1 levels were higher in patients with stages III-IV than in stages I-II,and the difference was statistically significant(t=41.269,P<0.05).LAG-3 levels were lower than in stages I-II,and the difference was statistically significant(t=39.627,P<0.05).Enhanced CT combined with serum DKK-1 and LAG-3had the highest diagnostic value for thyroid cancer and its staging,with statistically significant differences(P<0.05).Conclusion:Enhanced CT combined with serum DKK-1 and LAG-3 can improve the accuracy of thyroid cancer and its staging and synergistically enhance the diagnostic value.
