龈沟液中IL-8、MMP-8与急性牙髓炎患者根管治疗后疼痛程度的关系

Relationship Between IL-8,MMP-8 in Gingival Crevicular Fluid and the Degree of Pain After Root Canal Therapy in Patients with Acute Pulpitis

目的 探讨龈沟液中白细胞介素-8(IL-8)、基质金属蛋白酶8(MMP-8)与急性牙髓炎患者根管治疗后疼痛程度的关系。方法 回顾性收集2021年3月至2023年3月行根管治疗术的90例急性牙髓炎患者的临床资料,依据患者根管治疗后7 d时疼痛程度分为轻度疼痛组与中重度疼痛组。所有患者治疗前均完成龈沟液IL-8、MMP-8水平检测,比较两组龈沟液中IL-8、MMP-8水平以及一般资料,分析龈沟液中IL-8、MMP-8水平与急性牙髓炎患者根管治疗后疼痛程度的关系。结果 轻度疼痛组性别、年龄、体重指数、受教育程度、高血压、糖尿病与中重度疼痛组比较,差异无统计学意义(P>0.05);轻度疼痛组牙位、根充效果与中重度疼痛组比较,差异有统计学意义(P<0.05);轻度疼痛组龈沟液中IL-8、MMP-8水平均低于中重度疼痛组(P<0.05);经Kendall’s tau-b分析,龈沟液中IL-8、MMP-8水平与急性牙髓炎患者根管治疗后疼痛程度呈正相关(r>0,P<0.05)。结论 龈沟液中IL-8、MMP-8水平与急性牙髓炎患者根管治疗后疼痛程度具有密切关系。

Objective To investigate the relationship between interleukin-8(IL-8),matrix metalloproteinase 8(MMP-8)in gingival crevicular fluid and the degree of pain in patients with acute pulpitis after root canal therapy.Methods The clinical data of 90 patients with acute pulpitis who underwent root canal therapy from March 2021 to March 2023 were retrospectively collected.The patients were divided into mild pain group and moderate severe pain group according to their pain degree on the 7th day after root canal therapy.All patients completed the detection of IL-8 and MMP-8 levels in gingival crevicular fluid before treatment.The levels of IL-8 and MMP-8 in gingival crevicular fluid were compared between the two groups,as well as general data.The relationship between IL-8 and MMP-8 levels in gingival crevicular fluid and the degree of pain in acute pulpitis patients after root canal therapy was analyzed.Results There was no statistical significant difference in gender,age,body mass index,education level,hypertension,diabetes between mild pain group and moderate and severe pain group(P>0.05).The difference in tooth position and root filling effect between the mild pain group and the moderate to severe pain group was statistically significant(P<0.05).The average levels of IL-8 and MMP-8 in gingival crevicular fluid in the mild pain group were lower than those in the moderate to severe pain group(P<0.05).According to Kendall’s tau-b analysis,the levels of IL-8 and MMP-8 in gingival crevicular fluid were positively correlated with the degree of pain in patients with acute pulpitis after root canal therapy(r>0,P<0.05).Conclusion The levels of IL-8 and MMP-8 in gingival crevicular fluid are closely related to the degree of pain in patients with acute pulpitis after root canal therapy.
