纽曼系统护理模式联合营养支持对主动脉夹层患者术后自我效能及预后的影响

Effect of Newman System Nursing Model Combined with Nutritional Support on Self-efficacy and Prognosis of Patients After Aortic Dissection

目的 研究纽曼系统护理模式联合营养支持对主动脉夹层(AD)患者术后自我效能及预后的影响。方法 选取河南省胸科医院2019年12月至2021年12月收治的80例AD患者,采用随机信封法分为对照组和观察组,各40例。对照组接受常规护理及营养支持,观察组接受纽曼系统护理模式联合营养支持。比较两组血清营养指标[血红蛋白(Hb)、白蛋白(Alb)、转铁蛋白(TF)]、自我效能[自我效能量表(GSES)]、并发症发生率、临床指标及1 a病死率。结果 术后7、14 d观察组血清Hb、Alb、TF水平高于对照组(P<0.05),GSES评分高于对照组(P<0.05),并发症发生率低于对照组(P<0.05),卧床时间、肛门排气时间、住院时间短于对照组(P<0.05)。术后随访1 a,观察组病死率(2.56%)与对照组(7.69%)比较,差异无统计学意义(P>0.05)。结论 纽曼系统护理模式联合营养支持可改善患者营养状态,提高患者自我效能感,降低并发症发生率,促进康复。

Objective To study the effect of Newman system nursing combined with nutritional support on self-efficacy and prognosis of patients undergoing aortic dissection(AD).Methods Eighty patients with AD admitted to Henan Chest Hospital from December 2019 to December 2021 were randomly divided into control group and observation group,with 40 patients in each group.The control group received routine nursing and nutritional support,while the observation group received Newman systematic nursing mode combined with nutritional support.Serum nutritional indices[hemoglobin(Hb),albumin(Alb),transferrin(TF)],self-efficacy[self-efficacy scale(GSES)],complication rate,clinical indices and 1-year mortality of the two groups were compared.Results On the 7th and 14th day after operation,the levels of serum Hb,Alb and TF in the observation group were higher than those in the control group(P<0.05),the GSES score was higher than that in the control group(P<0.05),the incidence of complications was lower than that in the control group(P<0.05),and the bed rest time,anal exhaust time and hospitalization time were shorter than those in the control group(P<0.05).After a one-year follow-up,the mortality rate of the observation group(2.56%)was not significantly different from that of the control group(7.69%)(P>0.05).Conclusion Newman system nursing mode combined with nutritional support can improve patients’nutritional status,improve patients’sense of self-efficacy,reduce the incidence of complications,and promote patients’recovery and discharge.
