急诊高血压患者焦虑情绪与血压、心率变异性的关系

Relationship between anxiety and blood pressure and heart rate variability in acute hypertensive patients

目的 探讨急诊高血压患者焦虑情绪与血压、心率变异性的关系.方法 本研究观察对象为2020年1月~2022年4月84例急诊高血压患者,入院采用焦虑自评量表(SAS)评价焦虑情绪,设为焦虑组以及非焦虑组(38例与46例)比较两组患者24,h平均收缩压、舒张压(24h,SBP、24h,DBP),SBP、DBP波动值(ΔSBP、ΔDBP),检测心率变异性,包括相邻RR间期差值的均方根(RMSSD)、全部窦性心律(NN)R-R间期标准差(SDNN)、NN间期平均值的标准差(SDANN),并分析SAS评分与血压、心率变异性的相关性.结果 焦虑组24h,SBP、24h,DBP、ΔSBP、ΔDBP均显著高于对照组(P<0.05);焦虑组RMSSD、SDNN、SDANN均显著低于对照组(P<0.05);SAS评分与,24h,SBP、24h,DBP、ΔSBP、ΔDB呈正相关,与,RMSSD、SDNN、SDANN呈负相关(P<0.05).结论 急诊高血压患者焦虑情绪较为普遍,且焦虑情绪会增加血压升高幅度,降低心率变异性.

Objective To investigate the relationship between anxiety and blood pressure and heart rate variability in patients with acute hypertension.Methods In this study,84 patients with acute hypertension from January 2020 to April 2022 were observed.Anxiety was evaluated on admission using the Self-Assessment Scale for Anxiety(SAS)and was set up as anxiety group as well as non-anxiety group,38 cases versus 46 cases.The 24-h mean systolic and diastolic blood pressure(24-h SBP and 24-h DBP),SBP,DBP fluctuation range(△SBP and△DBP)were compared between the two groups.Heart rate variability was detected,including root mean square of the difference between adjacent RR in-tervals(RMSSD),standard deviation of all sinus rhythm(NN)R-R intervals(SDNN),standard deviation of mean NN intervals(SDANN).The correlation between SAS score and blood pressure and heart rate variability was also analyzed.Results 24h SBP,24h DBP,△SBP,and△DBP were significantly higher in the anxiety group than in the control group(P<0.05).RMSSD,SDNN,and SDANN were significantly lower in the anxiety group than in the control group(P<0.05).SAS scores were positively correlated with 24h SBP,24h DBP,△SBP,and△DB,and negatively cor-related with RMSSD,SDNN,and SDANN(P<0.05).Conclusion Anxiety is more common in patients with acute hy-pertension,and it increases the magnitude of blood pressure elevation and decreases heart rate variability.
