Effect of anxiety on Electrocardiogram in patients with myocardial infarction

目的 探讨合并焦虑症状对心肌梗死患者心电图的影响.方法 本研究选取2019年1月~2022年6月120例心肌梗死患者,根据焦虑症状评分情况设为焦虑症状组以及无焦虑症状组.所有患者均进行连续24,h动态心电图监测,记录室性早搏、心动过速等异常心电图情况,记录NN间期标准差(SDNN)、相邻NN间期差值均方根(rMSSD)等心率变异性(HRV)指标.结果 120例心肌梗死患者检出焦虑症状44例;焦虑症状组室性早搏、心动过速、室颤、房性早搏发生率显著高于无焦虑症状组(P<0.05),两组房颤、束支传导阻滞差异无统计学意义(P>0.05);观察组SDNN、rMSSD、差值>50ms的两相邻正常周期个数占比(PNN50)显著低于无焦虑症状组(P<0.05).结论 心肌梗死患者焦虑症状检出率较高,合并焦虑症状会增加心肌梗死患者异常心电图发生率,并降低患者HRV,影响自主神经功能.

Objective To explore the influence of anxiety on electrocardiogram in patients with myocardial in-farction.Methods A total of 120 patients with myocardial infarction from January 2019 to June 2022 were selected as the study subjects.They were divided into anxiety group and non anxiety group according to the level of anxiety.All patients underwent continuous 24-hour ambulatory electrocardiogram monitoring.The abnormal electrocardiogram con-ditions such as ventricular premature beats and tachycardia were recorded.The heart rate variability(HRV)indicators such as the standard deviation between NN intervals(SDNN)and the root mean square difference between adjacent NN intervals(rMSSD)were recorded.Results 44 cases of anxiety were detected in 120 patients with myocardial infarc-tion.The incidence of ventricular premature beats,tachycardia,ventricular fibrillation,and atrial premature beats in the anxiety group was significantly higher than that in the non anxiety group(P<0.05).There was no significant differ-ence in atrial fibrillation,and bundle branch block between the two groups(P>0.05).The proportion of SDNN,rMS-SD,and the number of adjacent normal cycles with a difference>50ms(PNN50)in the observation group was signifi-cantly lower than that in the non anxiety group(P<0.05).Conclusion The detection rate of anxiety in patients with myocardial infarction is high,and the combination of anxiety will increase the incidence of abnormal electrocardiogram in patients with myocardial infarction,reduce the patient's HRV,and affect autonomic nervous function.
