儿童口腔固定矫治前牙科畏惧症发生情况及其对患儿的影响

The incidence of dental fear before fixed orthodontic treatment and its influence in Children

目的 探讨儿童口腔固定矫治前牙科畏惧症发生情况及对治疗依从性、口腔健康生活质量的影响.方法 本研究纳入我院2019年6月~2022年3月90例牙颌畸形患儿,口腔固定矫治前采用Venham临床焦虑与合作行为级别评定量表(Venham)进行儿科牙科畏惧症评估,设为牙科畏惧症与对照组.比较两组治疗依从性、口腔健康生活质量.结果 90例学龄期牙颌畸形患儿共测出牙科畏惧症36例,Venham量表平均得分(3.15±0.30)分;牙科畏惧症组人际关系、自觉主动性、自律性、消极反抗性以及依从性总分均显著低于对照组(P<0.05);牙科畏惧症组功能、口腔、社会-情感、学校环境及自我形象以及口腔健康生活质量总分均显著低于对照组(P<0.05).结论 儿童口腔固定矫治前牙科畏惧症发生较高,牙科畏惧会降低口腔固定矫治治疗依从性以及口腔健康生活质量.

Objective To investigate the incidence of dental fear in children before fixed orthodontic treat-ment and its influence on treatment compliance and oral health quality of life.Methods Ninety children with dental malformations in our hospital from January 2020 to January 2022 were included in this study.Venham Clinical Anxiety and Cooperative Behavior Rating Scale(Venham)was used to evaluate pediatric dental fear before oral fixation treat-ment,and was set as dental fear group and control group.The treatment compliance and oral health life quality of the two groups were compared.Results Thirty six of 90 school age children with dental and maxillofacial deformities had den-tal fear,and the average score of Venham scale was(3.15±0.30).The total scores of interpersonal relationship,self-consciousness,initiative,self-discipline,passive resistance and compliance in the dental fear group were significantly lower than those in the control group(P<0.05).The total scores of functions,oral cavity,social emotion,school environment,self-image and oral health quality of life in the dental fear group were significantly lower than those in the control group(P<0.05).Conclusion Dental fear is higher in children before fixed orthodontic treatment.Dental fear will reduce the compliance of fixed orthodontic treatment and oral health quality of life.
