阶梯式心理护理对妇科恶性肿瘤患者负性情绪及生活质量的影响

Effect of stepped psychological nursing on negative emotion and quality of Life of patients with gynecological malignant tumors

目的 探讨阶梯式心理护理对妇科恶性肿瘤患者负性情绪及生活质量的影响.方法 本研究对象为我院2019年2月~2021年2月80例妇科恶性肿瘤患者,随机分为两组,两组均接受基础护理,对照组在基础护理基础上给予常规心理护理,观察组在基础护理上给予阶梯式心理护理.干预前后采用焦虑自评量表(SAS)、抑郁自评量表(SDS)评价患者焦虑及抑郁情绪,采用卡氏评分(KPS)评价患者治疗整体疗效,干预前后采用癌症患者生活质量核心问卷(EORTC,QLQ-C30)评估患者生活质量,记录不良反应发生率.结果 两组干预后较干预前SAS及SDS评分均显著下降,且干预后观察组两项评分均显著低于对照组(P<0.05),干预后两组KPS评分均显著升高,且治疗后观察组显著高于对照组(P<0.05);两组干预后较干预前EORTC,QLQ-C30量表中功能评分显著升高,症状评分显著下降,且干预后观察组功能评分显著高于对照组,症状评分显著低于对照组,(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05).结论 阶梯式心理护理能够显著降低妇科恶性肿瘤患者的焦虑、抑郁等负性情绪,并有助于提升其生活质量.

Objective To explore the effect of stepped psychological nursing on negative emotion and quality of life of patients with gynecological malignant tumors.Methods Eighty patients with gynecological malignant tumors in our hospital from February 2019 to February 2021 were the subjects of this study.They were randomly divided into two groups.Both groups received basic nursing.The control group was given routine psychological nursing on the basis of basic nursing,and the observation group was given ladder psychological nursing on the basis of basic nursing.Self rating Anxiety Scale(SAS)and self rating Depression Scale(SDS)were used to evaluate patients'anxiety and depres-sion before and after intervention,the curative effect of patients'treatment was evaluated by using Karnofsky(KPS).The quality of life of cancer patients was assessed by the core questionnaire of quality of life of cancer patients(EORTC QLQ-C30)before and after intervention,the incidence of adverse reactions was recorded.Results The SAS score and SDS score of the two groups after intervention were significantly lower than those before intervention,and the two scores of the observation group were significantly lower than those of the control group after intervention(P<0.05),after the intervention,the KPS scores of the two groups were significantly increased,and the observation group was significantly higher than the control group after treatment(P<0.05).After the intervention,the functional score in EORTC QLQ-C30 scale increased significantly and the symptom score decreased significantly.After the intervention,the functional score of the observation group was significantly higher than that of the control group,and the symptom score was significantly lower than that of the control group(P<0.05).There was no significant difference in the inci-dence of adverse reactions between the two groups(P>0.05).Conclusion Stepped psychological nursing can significantly reduce the negative emotions such as anxiety and depression of patients with gynecological malignant tumors,and help to improve their quality of life.
