食管癌手术患者重返工作后健康和劳动力现况及其与生活质量的相关性

目的探讨食管癌手术患者由于健康问题在有偿和无偿工作中的健康和劳动力状况,并分析其与生活质量的相关性。方法选取2021年1月至2022年12月河南省胸科医院胸外科食管癌患者为研究对象,对230例重返工作的食管癌手术患者应用一般资料调查表、癌症患者生命质量核心量表、健康和劳动力问卷进行调查。结果从事有偿工作的患者在过去两周内由于健康问题平均缺勤天数为(4.64±2.37)d,隐性缺勤平均分值为(13.17±3.91);患者付出无偿劳动平均时长为(19.14±8.63)h,因健康问题对无偿劳动影响程度平均分值为(4.18±1.89)分。重返工作小于6个月患者的缺勤天数及隐性缺勤的程度均高于重返工作6个月及以上者(t=2.229,P=0.031;t=3.243,P=0.002)。劳动障碍与生命质量综合维度呈负相关(r=-0.230,P=0.014),与疲劳呈正相关(r=0.212,P=0.023)。结论食管癌手术患者由于健康问题在有偿和无偿工作中存在不同程度的健康和劳动力损失,且患者生活质量越差越容易出现劳动障碍,应加大对食管癌幸存者重返工作后工作生存质量的关注力度。

Objective To explore the status of health and labour in patients with esophageal cancer surgery after returning to work,and analyze its correlation with the quality of life.Methods Esophageal cancer patients were selected in the thoracic surgery department of Henan chest Hospital,from January 2021 to December 2022.A general information questionnaire,the European Organization for Research and Treatment of Cancer quality of life QLQ-C30(EORTC QLQ-C30),the Health and Labor Questionnaire(HLQ)were delivered to 230 patients with esophageal cancer after returning to work.Results Among patients of paid labor,the average number of absenteeism days were(4.64±2.37)and(13.17±3.91)due to illness in the past two weeks.The average number of unpaid labor hours was(19.14±8.63)hours,and labor-related problems was(4.18±1.89).The scores of absenteeism days in patients returning to work less than 6 months were significantly higher(t=2.229,P=0.031;t=3.243,P=0.002).The labor-related impediments in esophageal cancer patients was negatively with global health status quality of life(r=-0.212,P=0.023),and positively correlated with fatigue symptom distress(r=-0.230,P=0.014).Conclusion The esophageal cancer patients had different degrees of productivity loss in paid and unpaid work due to health problems.The unhealthier the patients were,the more likely they had labor-related impediments.We should pay more attention to quality of life of patients with esophageal cancer in the future.
