钬激光碎石术联合坦洛新在输尿管结石的应用效果研究

Study on effect of Holmium laser lithotripsy combined with tanoxin in ureteric stones

目的分析钬激光碎石术联合坦洛新在输尿管结石的应用效果。方法选取2019年9月—2022年9月兰考第一医院输尿管结石患者100例,按随机数字表法分为观察组和对照组,各组50例。对照组予以钬激光碎石术治疗,观察组予以钬激光碎石术联合坦洛新治疗。对比两组围术期情况(手术时间、住院时间、结石排净时间)、疼痛视觉模拟量表(Visual Analogue Scale,VAS)评分、血管细胞黏附分子-1(vascular cell adhesion molecule-1,sVCAN-1)、血清皮质醇(cortisol,Cor)水平、炎性因子[白介素-6(Interleukin-6,IL-6)、肿瘤坏死因子(tumor necrosis factor,TNF-a)]水平及并发症。结果两组手术时间、住院时间对比无显著差异(P>0.05),结石排净时间较对照组短(P<0.05);术后7 d,观察组VAS评分较对照组低(P<0.05);术后7 d,观察组sVCAN-1、Cor水平较对照组低(P<0.05);术后7 d,观察组IL-6、TNF-a水平较对照组低(P<0.05);观察组并发症发生率8.00%(4/50)较对照组24.00%(12/50)低(P<0.05)。结论钬激光碎石术联合坦洛新治疗输尿管结石患者可缩短结石排净时间,减轻疼痛,降低sVCAN-1、Cor水平,减轻炎症反应,降低并发症发生率。

Objective To analyze the effect of Holmium laser lithotripsy combined with tanloxin in ureteral stones.Methods Totally 100 patients with ureteral stones in our hospital from September 2019 to September 2022 were selected and divided into observation group and control group according to random number table,with 50 cases in each group.The control group was treated with Holmium laser lithotripsy,and the observation group was treated with Holmium laser lithotripsy combined with tanloxin treatment.Comparing the perioperative period(operation time,hospital stay time,stone discharge time)and pain visual simulation scale(Visual Analogue Scale,VAS)score,vascular cell adhesion molecule-1(vascular cell adhesion molecule-1,The sVCAN-1),serum cortisol(cortisol,Cor)level,inflammatory factor[interleukin-6(Interleukin-6,IL-6),tumor necrosis factor(tumor necrosis factor,TNF-a)]levels and complications.Results There was no significant difference between operative time and hospital stay(P>0.05),shorter discharge time(P<0.05),7d after operation,lower VAS score(P<0.05),lower IL-6 and TNF-a(P<0.05);observation group complication rate 8%(4/50)is lower than that of control group 24%(12/50)(P<0.05).Conclusion Holmium laser lithotripsy combined with tanoxin treatment can reduce the stone discharge time,reduce pain,reduce sVCAN-1 and Cor levels,reduce inflammation,and reduce the incidence of complications.
