布地奈德结合维生素A为主的多种营养素预防早产儿支气管肺发育不良的分析

摘要 目的分析布地奈德结合维生素A为主的多种营养素预防早产儿支气管肺发育不良的效果。方法将商丘市第一人民医院在2021年1月至2022年12月期间收治的支气管肺发育不良早产患儿80例为研究对象,以随机数字表法分为观察组(n=40)、对照组(n=40),观察组实施布地奈德结合以维生素A为主的多种营养素治疗,对照组实施常规综合性治疗,对比两组患儿相关治疗时间、呼吸机参数、血气指标、炎性因子、营养状态以及副作用。结果观察组吸氧时间、住院时间、呼吸机停止使用时间短于对照组,组间对比统计学差异显著(P<0.05)。治疗后,观察组患儿呼气末正压(PEEP)、吸气峰压(PIP)、吸入氧浓度(FiO_(2))水平低于对照组,血氧分压(PaO_(2))水平高于对照组、二氧化碳分压(PaCO_(2))水平低于对照组,炎性因子水平低于对照组,组间对比差异有统计学意义(P<0.05)。两组患儿并发症发生率对比无显著差异(P>0.5),但观察组并发症发生率稍低。结论针对支气管肺发育不良早产患儿实施布地奈德结合以维生素A为主的多种营养素进行预防性治疗,对改善患儿呼吸机参数、优化血气参数具有积极作用,对降低患儿炎性因子水平、缩短治疗时间、提升患儿营养状态具有重要作用,联合治疗对患儿产生的副作用不显著,治疗安全性较高,值得推广。 Objective To analyze the effects of budesonide combined with vitamin A in preventing bronchopulmonary dysplasia in premature infants.Methods Totally eighty premature children with bronchopulmonary dysplasia admitted to our hospital from January 2021 to December 2022 were divided into observation group(n=40)and control group(n=40)by random number table method.The observation group was treated with budesonide combined with multiple nutrients mainly vitamin A,while the control group was treated with conventional comprehensive treatment.The duration of treatment,ventilator parameters,blood gas indexes,inflammatory factors,nutritional status and side effects were compared between the two groups.Results The oxygen inhalation time,hospital stay and ventilator stop time of the observation group were shorter than those of the control group,and the difference between groups was statistically significant(P<0.05).After treatment,positive end expiratory pressure(PEEP),peak inspiratory pressure(PIP)and inhaled oxygen concentration(FiO_(2))in the observation group were lower than those of the control group,partial oxygen pressure(PaO_(2))was higher than those of the control group,partial carbon dioxide pressure(PaCO_(2))was lower than those of the control group,and inflammatory factors were lower than those of the control group.There was statistical significance in comparison between groups(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.5),but the incidence of complications in the observation group was slightly lower.Conclusion Prophylactic treatment of budesonide combined with various nutrients mainly vitamin A for premature children with bronchopulmonary dysplasia has positive effects on improving ventilator parameters and optimizing blood gas parameters of the children,and plays an important role in reducing inflammatory factors,shortening treatment time and improving nutritional status of the children.The side effects of combined treatment are not significant.The treatment is safe and worthy of promotion.
