Prognostic Value of C-reactive Protein to Albumin Ratio in COVID-19 Patients

目的:分析COVID-19患者疾病严重程度和死亡资料,评估C-反应蛋白与白蛋白比值(CAR)对COVID-19患者的预后价值。方法:选择2022年12月—2023年1月在我院就诊的COVID-19患者进行研究,根据疾病严重程度将COVID-19患者分为非重症组与重症组,根据预后情况又将COVID-19患者分为生存组与死亡组,收集COVID-19患者入院时首次CRP与Alb检测结果进行分析。结果:重症组男性占比、年龄、CRP与CAR水平明显高于非重症组,Alb水平明显低于非重症组。生存组与死亡组性别、年龄比较差异无统计学意义,死亡组CRP与CAR水平明显高于生存组,死亡组Alb水平明显低于生存组。进行Logistic回归分析,发现年龄、CRP、CAR是COVID-19疾病严重程度的危险因素;CRP、CAR是COVID-19疾病死亡的危险因素。在预测COVID-19疾病严重程度时,CAR的ROC曲线下面积(AUC值)最大,在预测COVID-19疾病死亡时,CAR的AUC值仍为最大。结论:CAR是COVID-19感染相关严重程度和死亡率的良好预测指标。

Objective:To analyze the disease severity and death data of COVID-19 patients,and evaluate the prognostic value of C-reactive protein to albumin ratio(CAR)for COVID-19 patients.Methods:The COVID-19 patients who were treated in our hospital from December 2022 to January 2023 were selected for the study.According to the severity of the disease,the COVID-19 patients were divided into the non-critical group and the critical group.According to the prognosis,the COVID-19 patients were divided into survival group and death group.The first CRP and Alb test results of the COVID-19 patients were collected for analysis.Results:The proportion of men,age,and the level of CRP and CAR in the critical group was significantly higher than that in the non-critical group,and the level of Alb in the critical group was significantly lower than that in the non-critical group.There was no significant difference in sex and age between the survival group and the death group.The level of CRP and CAR in the death group was significantly higher than that in the survival group,while the level of Alb in the death group was significantly lower than that in the survival group.Logistic regression analysis showed that age,CRP and CAR were risk factors for the severity of COVID-19 disease;CRP and CAR are risk factors for COVID-19 disease death.When predicting the severity of COVID-19 disease,the area under the ROC curve(AUC value)of CAR is the largest.When predicting the death of COVID-19 disease,the AUC value of CAR is still the largest.Conclusion:CAR is a good prognostic indicator of the severity and mortality associated with COVID-19 infection.
