NLR、PLR、LMR水平预测2型糖尿病患者白蛋白尿短期进展的价值

Value of NLR,PLR and LMR levels in prediction of short-term progression of albuminuria in patients with type 2 diabetes mellitus

目的:分析中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、淋巴细胞/单核细胞比值(LMR)水平预测2型糖尿病患者白蛋白尿短期进展的价值。方法:回顾性分析2016年1月至2019年1月该院重复收治的162例2型糖尿病患者的临床资料,按患者2次入院是否存在白蛋白尿短期进展分为进展组(n=42)和无进展组(n=120)。比较两组第1次入院时NLR、PLR、LMR水平;采用受试者工作特征(ROC)曲线分析第1次入院时NLR、PLR、LMR水平对2型糖尿病患者白蛋白尿短期进展的预测价值。结果:第1次入院时,进展组NLR、PLR水平均高于无进展组,LMR水平低于无进展组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,第1次入院NLR、PLR、LMR水平联合检测预测2型糖尿病患者白蛋白尿短期进展的价值较高(曲线下面积=0.908),且高于各单项检测。结论:NLR、PLR、LMR水平联合检测预测2型糖尿病患者白蛋白尿短期进展的价值较高,且高于各单项检测。

Objective:To analysis value of neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR)and lymphocyte-to-monocyte ratio(LMR)in prediction of short-term progression of albuminuria in patients with type 2 diabetes mellitus.Methods:The clinical data of 162 patients with type 2 diabetes mellitus who were repeatedly admitted to the hospital from January 2016 to January 2019 were retrospectively analyzed.These patients were divided into progression group(n=42)and non-progression group(n=120)according to whether there was short-term progression of albuminuria after second admissions.The levels of NLR,PLR and LMR at the first admission were compared between the two groups.ROC curve was used to analyze the predictive value of NLR,PLR and LMR levels at the first admission on the short-term progression of albuminuria in the patients with type 2 diabetes mellitus.Results:At the first admission,the levels of NLR and PLR in the progressive group were higher than those in the non-progressive group,the level of LMR was lower than that in the non-progressive group,and the differences were statistically significant(P<0.05).The ROC curve showed that the combined detection of NLR,PLR and LMR levels at the first admission had a higher value in the prediction of the short-term progression of albuminuria in the patients with type 2 diabetes(AUC=0.908),which was higher than that of each single test.Conclusions:The combined detection of NLR,PLR and LMR levels has a higher value in the prediction of the short-term progression of albuminuria in the patients with type 2 diabetes mellitus,and is higher than each single test.
