房水细胞因子水平与糖尿病性白内障术后发生黄斑水肿的相关性

Correlations among cytokine levels in aqueous humor and macular edema after diabetic cataract surgery

目的:探讨房水细胞因子水平与糖尿病性白内障术后发生黄斑水肿的相关性。方法:选取2020年3月至2022年12月该院收治的70例行手术治疗的糖尿病性白内障患者进行前瞻性研究,术后随访6周,统计术后黄斑水肿的发生情况,并依据是否发生黄斑水肿将其分为发生组与未发生组;纳入同期30名健康体检者作为对照组,比较三组房水细胞因子[血小板衍生生长因子(PDGF)、血管内皮生长因子(VEGF)、白细胞介素-6(IL-6)、内皮抑制素(ES)]水平,采用点二列相关性分析房水细胞因子水平与糖尿病性白内障患者术后发生黄斑水肿的相关性。结果:随访6周,70例糖尿病性白内障患者术后发生黄斑水肿25例,占比35.71%(25/70);未发生黄斑水肿45例,占比64.29%(45/70);未发生组与对照组房水PDGF、VEGF、IL-6、ES水平比较,差异无统计学意义(P>0.05);发生组PDGF、VEGF、IL-6水平均高于未发生组、对照组,ES水平低于未发生组、对照组,差异有统计学意义(P<0.05);经二列相关性分析结果显示,PDGF、VEGF、IL-6水平与糖尿病性白内障患者术后发生黄斑水肿呈正相关(r>0,P<0.05),ES水平与糖尿病性白内障患者术后发生黄斑水肿呈负相关(r<0,P<0.05)。结论:PDGF、VEGF、IL-6水平与糖尿病性白内障患者术后发生黄斑水肿呈正相关,ES水平与糖尿病性白内障患者术后发生黄斑水肿呈负相关。

Objective:To investigate correlations among cytokine levels in aqueous humor and macular edema after diabetic cataract surgery.Methods:A prospective study was conducted on 70 patients with diabetic cataract who underwent surgical treatment in this hospital from March 2020 to December 2022.These patients were followed up for 6 weeks.The occurrence of postoperative macular edema was statistically analyzed.According to whether macular edema occurred,they were divided into occurrence group and non-occurrence group.In the same period,30 healthy subjects were included as control group.The levels of cytokines[platelet-derived growth factor(PDGF),vascular endothelial growth factor(VEGF),interleukin-6(IL-6),endostatin(ES)]in aqueous humor were compared among the three groups.The correlations among the the level of cytokines in aqueous humor and macular edema after diabetic cataract surgery were analyzed by point biserial correlation analysis.Results:After 6 weeks of follow-up,25 cases of macular edema occurred in the 70 patients with diabetic cataract,accounting for 35.71%(25/70);there were 45 cases without macular edema,accounting for 64.29%(45/70).There were no significant differences in the levels of PDGF,VEGF,IL-6 and ES in aqueous humor between the non-occurrence group and the control group(P>0.05).The levels of PDGF,VEGF and IL-6 in the occurrence group were higher than those in the non-occurrence group and the control group;the ES level was lower than that of the non-occurrence group and the control group;and the differences were statistically significant(P<0.05).The point biserial correlation analysis showed that the levels of PDGF,VEGF and IL-6 were positively correlated with postoperative macular edema in the diabetes cataract patients(r>0,P<0.05),and the level of ES was negatively correlated with postoperative macular edema(r<0,P<0.05).Conclusions:The levels of PDGF,VEGF and IL-6 are positively correlated with postoperative macular edema in the patients with diabetic cataract,and the level of ES is negatively correlated with postoperative macular edema.
