坐、卧位两种体位下手法治疗肩周炎的疗效比较

Comparative of Therapeutic Effects of Manipulations in Sitting and Lying Positions on Scapulohumeral Periarthritis

目的研究坐、卧位两种体位对手法治疗肩周炎疗效的影响。方法将肩周炎患者随机分为卧位治疗组(试验组)、坐位治疗组(对照组),每组35例,两组患者均每周一、周四(或周二、周五)各治疗1次,共治疗8周。于治疗期结束后1个月进行随访,比较两组治疗后、随访时的视觉模拟量表(VAS)评分、肩关节疾患治疗成绩判定标准[日本骨科协会评估治疗(JOA)]评分,通过《中医病证诊断疗效标准》中的肩周炎治疗效果评定标准判断两组患者的疗效。结果治疗后及随访时,两组VAS评分均低于治疗前,且治疗组低于对照组(P<0.05)。治疗后及随访时,两组肩关节JOA评分均高于治疗前,且治疗组高于对照组(P<0.05)。试验组治愈17例,好转18例,总有效率为100%;对照组治愈6例,好转24例,总有效率为85.71%。结论与坐位治疗相比,卧位下手法治疗肩周炎的效果更好。

Objective To study the effect of sitting and lying positions on the treatment of periarthritis of shoulder with manipulation.Methods Patients with scapulohumeral periarthritis were randomly divided into a recumbent treatment group(experimental group)and a sitting treatment group(control group),with 35 patients in each group.The patients in both groups were treated once a week,once a Thursday(or two or five),for a total of 8 weeks.A follow-up was conducted one month after the end of the treatment period to compare the pain Visual Analogue Scale(VAS)scores and the shoulder joint disease treatment performance evaluation standard[Japanese Orthopedic Association Scores(JOA)]scores of the two groups after treatment and during the follow-up.The efficacy of the two groups of patients was judged by the treatment efficacy evaluation standard for periarthritis of the shoulder in the diagnosis and efficacy standard of traditional Chinese medicine.Results After treatment and during follow-up,the VAS scores in both groups were lower than those before treatment,and the experimental group was lower than the control group(P<0.05).After treatment and during follow-up,the JOA scores of shoulder joint diseases in both groups were higher than those before treatment,and the experimental group was higher than the control group(P<0.05).In the experimental group,17 cases were cured and 18 cases were improved,with a total effective rate of 100%.In the control group,6 cases were cured and 24 cases improved,with a total effective rate of 85.71%.Conclusion Manipulation therapy in the recumbent position is more effective compared than the sitting treatment.
