社会疏离在中青年脑卒中患者领悟社会支持与负性情绪间的中介效应

Mediating effect of social alienation on the perceived social support and negative emotion in young and middle-aged stroke patients

目的探讨社会疏离在中青年脑卒中患者领悟社会支持与负性情绪间的中介效应,为改善其负性情绪提供新方法。方法采用便利抽样法,选取2021年9月-2022年2月湖北省某三级甲等综合医院神经内科住院及神经内科门诊就诊的350例中青年脑卒中患者为研究对象,使用一般情况调查表、领悟社会支持量表(perceived social support scale,PSSS)、一般疏离感量表(general alienation scale,GAS)和医院焦虑抑郁量表(hospital anxiety and depression scale,HADS)进行调查。运用Pearson相关分析法分析领悟社会支持、社会疏离与负性情绪间的相关性;采用Process宏程序的Bootstrap抽样5000次进行中介效应分析和检验。结果304例患者完成研究。相关分析结果显示,领悟社会支持与社会疏离、负性情绪呈负相关(r=-0.743、-0.652,均P<0.01),社会疏离与负性情绪呈正相关(r=0.738,P<0.01)。领悟社会支持对负性情绪存在直接效应(β=-0.130,95%CI:-0.193~-0.068),并通过社会疏离的部分中介效应间接影响负性情绪(β=-0.236,95%CI:-0.299~-0.183),中介效应占总效应的64.48%。结论社会疏离在中青年脑卒中患者领悟社会支持和负性情绪间发挥部分中介效应,医护人员应关注中青年脑卒中患者的社会疏离水平,采取干预措施减少其社会疏离,提高其领悟社会支持水平,从而改善负性情绪。

Objective To explore the mediating effect of social alienation on the perceived social support and negative emotion in young and middle-aged stroke patients,and to provide new thoughts in mitigation of negative emotions.Methods A convenience sampling method was used to select 350 young and middle-aged stroke patients as the study population.The selected patients included both of inpatients and outpatients,from September 2021 to February 2022,of the Department of Neurology of a tertiary general hospital in Hubei Province.General information questionnaire,perceived social support scale(PSSS),general alienation scale(GAS)and hospital anxiety and depression scale(HADS)were employed in the investigation.Pearson correlation analysis was carried out to explore the correlations of the perceived social support,social alienation and their dimensions with the negative emotions.Bootstrap of Process macro program was used to analyse and verify the effect of mediation.Results A total of 304 patients had completed the investigation.By correlation analysis,it was found that the social alienation and negative emotions were negatively correlated with the perceived social support(r=-0.743,-0.652,both P<0.01),while the social alienation was positively correlated with the negative emotions(r=0.738,P<0.01).It was also found that perceived social support had direct effects on the negative emotions(β=-0.130,95%CI:-0.193~-0.068),with indirect effect on the social alienation on negative emotions(β=-0.236,95%CI:-0.299~-0.183).The indirect mediating effect was accounted for 64.48%of the total effect.Conclusions Social alienation plays a partial mediating role between a perceived social support and negative emotions in young and middle-aged stroke patients.Medical staff should pay attentions to the level of social alienation in young and middle-aged stroke patients,take intervention measures to reduce the social alienation and improve the level of perceived social support,therefore to minimise the negative emotions.
