摘要
体外二氧化碳清除技术(extracorporeal carbon dioxide removal,ECCO_(2)R)旨在去除血液中二氧化碳,以缓解高碳酸血症和相关酸中毒的不良影响,为保护性甚至超保护性机械通气提供条件,有助于减少有创呼吸机使用,缩短呼吸机带机时间。近年来,该技术在急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)、慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)、新冠肺炎及等待肺移植手术等患者中发挥了重要作用。本文就近年来国内外ECCO_(2)R的临床应用进展做一述评,以供临床进一步推广提供参考与借鉴。
Extracorporeal Carbon dioxide removal(ECCO_(2)R)is designed to remove carbon dioxide from the blood to alleviate the adverse effects of hypercapnia and related acidosis.It provides conditions for protective or even super-protective mechanical ventilation,helping to reduce the use of invasive ventilators and shorten the duration of mechanical ventilation.In recent years,the technique has played an important role in acute respiratory distress syndrome(ARDS),acute exacerbation of chronic obstructive pulmonary disease(AECOPD),COVID-19 and patients waiting for lung transplantation.This article reviews the progress of clinical application of ECCO_(2)R at home and abroad in recent years,and provide reference for its further clinical application and promotion.
